  2. 3rd party vitalchek

    Ordered birth certificate on the 17th of August. Was told to keep checking the mail. It was sent to you on the 24th of August. Today is the 19th of Sept. AZDHS website states for faster service we use 3rd party services. It is so inconvenient and such a headache. I get nowhere talking with anyone. Told to go to the website and click on the big red link and I would be able to request another copy for free of charge. To no surprise, the link went to vitalchek so I can order a new one for a fee.…

  3. list approved alcohol agency

    my wife has dui. we cannot figure out how to get list of approved alcohol counseling agencies on line. just want a list...cannot figure website out. we are old and do not understand computers very well.

  9. Publish a list of doctors that will see patients seeking aMMC card

    Attempting to find a list of physicians able to help a patient obtain a MMC card. My primary care physician doesn't know of anyone in AZ.

    I can't get into your system and no one will help me. I called twice and got a recording and can't get in to take care of a death certificate. Please make this system much easier for doctors to use. I can't even sign in.

  11. Need better response and follow thru on a submitted complaint

    I have submitted a complaint ID 2018-MED1200 and I have not received any response to who, what and how they are reviewing my concerns. No one has contacted me or my family. This is a very serious complaint and I feel like no one is responding.

  14. Vitalchek Service

    I think you that you should mention that vitalchek charges $49 to essentially fill out a form that can just as easily be done directly from the state website. You will still have to pay the state fees. I am so disappointed with this lack of disclosure. What could have cost $33 will end up costing $82.

  15. Eduprize Gilbert location isn't in the school immunization coverage document

    Eduprize has two campuses and the one that is located in Gilbert at 580 W Melody Ave. was not in the School Immunization Coverage Document. I don't know if this is on the school or ADHS, but can it be included in future years? Thank you!

  16. Please update CHA/CHIP for all counties - they are all too old.

    On this page: https://www.azdhs.gov/operations/managing-excellence/index.php#ship-sha-community.
    All County CHA/CHIPs are from 2012-13. Only Cochise shows a current one - 2017. Please update this page with the most current County CHA/CHIPs.

    My wife, Francisca Amada Acuna was born in the Ajo Hospital on 3 August, 1965. Would that birth have been automatically registered at your central records office for births, deaths and marriages? If so, how do we apply for a copy of the birth record, please?
    That hospital in Ajo was closed down many years ago so if there is no central recording office for the State, where are those hospital records archived so that a copy of the original birth certificate can be accessed?

  19. tip: Guitar Center at Peoria & I-17 had flood-- possible mould.

    anonymous tip-- if you're interested.. possible mould danger at Guitar Center, I-17 and Peoria from massive roof leak on Mon-- store still open Tues while workmen clearing mess through the store!? -- I think that that's not right-- they should close the store for a few days right? Very damp in sections of store-- not right. Please accept my anonymouty. Thanks. (by the way-- a suggestion for this box is to have a button for hot tips on your front web page)
