    My computer does not load enable, play, do, whatever so I can pay for my address change onmy card. I do nor have SASFARUI I do not have chrome it ewill not losad.   4. getting 404 page not found at https://ems.azdhs.gov/Application/CreateEligibility

    I'm attempting to apply for my state EMT certification, I am already have my NREMT. I am trying to apply for jobs and the card from the state is all I am missing and cannot move forward till I have it. After entering my information and cliciking next on this page, https://ems.azdhs.gov/Application/CreateEligibility - it takes me to a page saying Error 404, file or directory not found and I cannot complete my application.

  5. Flu vaccine doesn't always work

    Please explain to the public why some of us get the flu even after we have received the flu vaccine. Flu was just confirmed at physician's office. This is of concern since the past 3 years since I moved to Maricopa county, I have received the vaccine each year and each year I get sick with the flu. I doubt i am the only person in this state with this problem

    I tried to email you about a dirty restaurant but you have not made it easy, it looks like you do not have this ability. May I suggest you make it possible, as for phoning you are closed when I am able to call.
    Not sure if this is even the right place for my concern.

    My name is Jin-young KIM.
    I'm a Korean.
    I am going to study at Arizona State University.
    I have two elementary school students.

    My daughter is 7 years old and 9 years old.

    It is hoped the elementary school on January 07, 2015.

  14. I requested an application for a copy of my birth certificate via the phone & have not received it yet.

    please make it easier to print an application for a copy of a birth certificate. The Microsoft Reader wouldn't open for me.

  18. I was given a map that depicts the Brth/Death Department location.

    Invert the picture of the location. It appears to be upside down.

  19. It is impossible to follow a Chapter/Article from index to actual reference.

    Try Chapter 10, Article 8. Can you locate Article 8? Or, click on one of the high-lited lines in the cover Index; Your sub-pages [if there is a sub-page] do NOT reference the Chapter ! This is so poorly done, I am in shock. Surely the Az Dept of Health Services can find somebody who can do a better job than this !!!

    Thank you for your comments. The Department is looking into revising the webpages.
    If you need the information, To get to the information now, click on Chapter 10 at the top and that will take you to the Articles that you can click on for the document you are referencing.
    If you still have problems, please call the ADHS main number at ADHS: 602 542-1025.
