complaint form suggestions
Hi:
1. your complaint form should issue a email confirmation it was received complete with complaint number, copy of message sent, and information as to what happens next.
2. your complaint form should have a way to attach documents
3. this service should include updates so you can view the progress of the complaint.

A DHB from Arizona is taking 6.77 out of my checking account monthly. I did not authorize this. Please cease and desist immediately.
My bank and I have reason to believe this is fraudulent.I don't have an account with you.
