  2. Concerns with prescriptions for opioids

    I have been on a opioid prescription for 3 years and it is one of the few of many I have tried that works. My dosage was cut in half because of the people abusing it. It is wrong the I have to suffer in pain because of other peoples abuse! Something needs to be done about this because their are many more people who are paying for it too. I have Lyme's disease and arthritis and some days are better that others but without the meds I take my quality of life would be really really bad! I hope…

  4. Error CORRECTIONS

    I am an almost daily user of your data base, but today I came across a mess. All entries for Pima County, 1912 Deaths, numbers 33 through 48 are incorrect names and they are from Maricopa and Mohave counties. Are there coresponding entries elsewhere that are correct for Pima County?
    As an aside, when searching for any given year in Pima County (and I assume each county has similiar entries), there are about 30 inirial entries that are the same for any given year search that have incorrect dates which anyone with access to your system could correct in a…

  5. Convert the BRFSS survey to digital please!

    I participated in the BRFSS by phone today and found the process to be almost unbearably painful. As a health care provider I appreciate the value of the data collected, but doing it as a scripted phone interview was overly long and difficult. I was able to find and read the questions on-line and should be able to complete the task more accurately and in a fraction of the time electronically.   9. Why full price of $ 300 for renewal with incurable diseases

    Don t punish patients with expiring card and an ongoing incurable disease like Parkinson's with a $ 150 fee from a doctor who is not even the patients physician. He has also developed another cancer for which he is paying $ 2000 for the radiation. the products in dispensaries are expensive enough. Why can his physician/oncologist NOT write up the health condition and reason for a medical marijuana card yo avoid that fee? It is really unfair, just an additional income for doctors.   10. Hire a completely new staff

    I can get better customer service from a lobster in the middle of the Atlantic ocean in the pit of a whale shark's stomach that has croaked over and been eaten by 19 great white sharks. Your customer services are worse than a banana peel that has been sitting in the garbage can in my bathroom for the last 7 months. FIX IT

  13. Fraudulent Billing Information

    When I submitted my insurance information for my newborn's screening test, I received a bill from ADHS saying that the patient could not be identified by the insurance as insured. I checked with my third-party administrator and found out that the claim had been accepted and was being processed. When I called your billing department to find out why that note had been added, the only response I got was that my third-party administrator "takes a long time" and that a note would be added to my account, after which I was hung up on.   17. Cannot FIND WAYS to select/specify a designated CARE GIVER

    I live in Kearny, Arizona. Superior Organics Dispensary CLOSED. I would like to select a DESIGNATED CAREGIVER as a supplier. But I CANNOT FIND THE APPLICATION FORM that I need to fill out to identify and select my Designated Care Giver. Can you help?

    Hi:
    1. your complaint form should issue a email confirmation it was received complete with complaint number, copy of message sent, and information as to what happens next.
    2. your complaint form should have a way to attach documents
    3. this service should include updates so you can view the progress of the complaint.

  20. Smoking Violations Submittal - does not confirm success

    Your Android and regular computer submission process does not confirm whether or not submission was successfully submitted - I tried numerous times and on multiple applications to submit my concern/complaint - could you please confirm that my concerns were received?
