  1. Status on MMJ card delivery?

    I’m not sure if this has happened to anyone else but I received an email stating my application was accepted on february 17th and I still have yet to receive my medical card? Is there any way to see what’s going on with my card? All I see are tons of different phone numbers for contact but none of them explicitly state they’re for MMJ patients.

  2. Abuse of the public market. The cost for vital records, death , birth certificated is too high.

    Find another vendor that is less than $5.00 ( not $11.00) for online vital records documents and decrease the cost of the AZ document to $12.00. This service must get under $20.00 where it is not a financial hardship to patrons for this public service! Joyce A. Brown 15Jan2019

  3. Where is my MMJ card? I sent in the correction requested 2/1. All the system will tell me is that my correction was received!

  4. update how you apply and your links

    Make it Mac friendly!! Make it so you can apply in person or through the mail I am trying to submit a better pic(You emailed me to tell me you needed a brighter pic) and the I type in my ID, name, DOB and it just sits there, I called someone and they told me it wasn't Mac friendly so I took time off work to go to the library and I'm having the same issue. I need to mail a pic in or bring one in, not very PC if you can't even do it in person! And I…

  5. WHY HAS MY CONPLAINT BEEN IGNORED

    Iwas falsly acused of abusing my mother by a hospice worker it took 3yrs to get the report it turned out i was in the rite and now nobody will do any thing about it I can not explain how it feels to be acused of abusing my mother it,s a real shame that the state let.s it happen i want some one to respond to this complaint

  7. add a link to verify licensure of medical professionals on your home page!

    as stated above. I cannot find the info that you state elsewhere is public. I tried to submit online form emails twice on those sites, and they both failed for no apparent reason - the error said something like there was an invalid character in the subject line, of which there is none. No subject line. Can't correct what isn't there (or visible to me, anyway). where do I get verification of a doctor's license to practice in AZ?? thank you.

  9. Link to Hospital Chargemasters on Home Page

    How about putting current hospital chargemasters on the Home Page of your website if they are anywhere on the website at all. I found OLD DATA from 2012 comparing hospitals. USELESS. Chargemasters are now required by federal law. You are doing nothng to simplify the search for this information.

  18. My experience with the Arizona Office of Vital Statistics has been terrible. I am an attorney in Michigan who has been trying for 3 weeks

    to get a death certificate for an Arizona decedent. I am aware that Arizona is a closed records state. I reviewed Arizona law and administrative rules before sending a detailed written request setting forth my clients’ eligibilty (to clear title to jointly held property in Michigan). After 3 weeks I got a “form” deficiency letter that was not specific to my detailed request. A letter faxed to the supervisor went unanswered. Please help me. Randy Martinek, Esq. Cell: 810-434-3743.   20. Please make it easier for physicians to find the log in to register a death certificate. I have to search too long to find it
