  3. You do not help much...expensive tests and long waits to hear results, no tests for the lack of nutrients, no supplements to help the bo

    In the ER and generally, Iv's of Vitamin C, other vitamins and minerals to help the immune system respond to whatever is happening to a patient.

  4. return genealogy death and birth records

    As a 4th generation AZ, i have found the online search for early birth and death records very valuable...but am unable to locate those records now. please return them online...thank you!

  6. I am being ignored by the City of Sedona and Coccino County

    I have been suffering headaches and nasal congestion for months because Cocconino County will not put out fires!! It is said that the fires cannot be reached, but no airplane water release has been used. California would not allow a fire to just simmer for weeks on end!! Sedona residents have suffered from smoke related problems for most of the summer continuing into the fall with no end in sight. Health issues are being ignored by local government! Can I sue the state for such neglect?

  7. problems with expedited service for birth certificates

    I went thru Vital Chek for expedited service, nothing but a joke, they kept saying they needed more proof of ID I supplied them with (BC, drivers license, and military ID), I never received my certificate for my son but they sure enough charged my card for the service, WHAT A JOKE

  9. 3rd party vitalchek

    Ordered birth certificate on the 17th of August. Was told to keep checking the mail. It was sent to you on the 24th of August. Today is the 19th of Sept. AZDHS website states for faster service we use 3rd party services. It is so inconvenient and such a headache. I get nowhere talking with anyone. Told to go to the website and click on the big red link and I would be able to request another copy for free of charge. To no surprise, the link went to vitalchek so I can order a new one for a fee.…

    my wife has dui. we cannot figure out how to get list of approved alcohol counseling agencies on line. just want a list...cannot figure website out. we are old and do not understand computers very well.

  11. Can you check Madison street jail for funguses or any type diseases growing on the floors?

    There are many people I know that have came up with many different types of foot fungus and also a variety of diseases of many sorts. Some of them include Hepatitis and also MRSA. Can we have the floors and even the water tested for diseases. I only came up with a terrible foot fungus or athletes foot for at least 9 months. A concerned and innocent person that had to stay there for a night or so. I was innocent but later agreed to stupid charges. Can we have to closed down jail tested. I think it would be…

  17. Publish a list of doctors that will see patients seeking aMMC card

    Attempting to find a list of physicians able to help a patient obtain a MMC card. My primary care physician doesn't know of anyone in AZ.

    I can't get into your system and no one will help me. I called twice and got a recording and can't get in to take care of a death certificate. Please make this system much easier for doctors to use. I can't even sign in.

  19. Need better response and follow thru on a submitted complaint

    I have submitted a complaint ID 2018-MED1200 and I have not received any response to who, what and how they are reviewing my concerns. No one has contacted me or my family. This is a very serious complaint and I feel like no one is responding.
