Vitalchek Service
I think you that you should mention that vitalchek charges $49 to essentially fill out a form that can just as easily be done directly from the state website. You will still have to pay the state fees. I am so disappointed with this lack of disclosure. What could have cost $33 will end up costing $82.

Eduprize Gilbert location isn't in the school immunization coverage document
Eduprize has two campuses and the one that is located in Gilbert at 580 W Melody Ave. was not in the School Immunization Coverage Document. I don't know if this is on the school or ADHS, but can it be included in future years? Thank you!

tip: Guitar Center at Peoria & I-17 had flood-- possible mould.
anonymous tip-- if you're interested.. possible mould danger at Guitar Center, I-17 and Peoria from massive roof leak on Mon-- store still open Tues while workmen clearing mess through the store!? -- I think that that's not right-- they should close the store for a few days right? Very damp in sections of store-- not right. Please accept my anonymouty. Thanks. (by the way-- a suggestion for this box is to have a button for hot tips on your front web page)

Recently Added MMJ Law
Hoping to at least regulate opiates if we are regulating MMJ this ruthlessly. MMJ concentrates have never robbed someone of life- on the other hand, opiates? Opiates have robbed thousands of life. Folks within my family and dear friends close to our family have been demoralized, sick and dead. I do not understand the effort and the amount of energy going toward into this law- I hope the department of health can maintain a balanced outlook and considers regulations on all forms of medication.

If you choose to let your card expire can you renew with th renewal application?
All it says in the rules and regs is it you allow your card to expire and are renewing you have to fill out the original app. Why can't we fill out the regular renewal form. Also it's not the doctors fault if you allowed the card to expire. Why would their card number expire too?

Additional information and support
I have reviewed this act many times and about confidentiality and protection. With concerning to pre-employment screenings for jobs, speaking with medical review officer and keeping the confidentiality as this law states. It would be helpful to have information and walk through of what this process should look like and resources for legal council that can answer questions and support a person with their rights.

Dysfunctional Application and Broken Email Link
Trying to use Cancer Cluster map by Neighborhood, and the application does not open. The link to leave feedback on the application is broken. This is important to me; please fix. I already updated the Adobe Flash. That is not the issue.

Trauma Surgical Facilities
In recent years, Account Pamela & Associates conducted a feasibility study on Trauma Surgical Facilities in Arizona. More recent research reveal the southern areas of Phoenix are without adequate medical facilities and hospitals. Specifically, the Ahwatukee and South Mountain area is currently observed for Trauma Surgical Facilities. FYI: The nearest hospital in some locations are between 10 and 17 miles away. Account Pamela & Associates in coming months will propose Trauma Surgical Facilities and other Life Care Facilities for the growing areas of Arizona.

our contact section refuses to send my message stating that cookies are disabled. That is not so. Cookies are enabled so your tech needs to get to work and fix whatever problem your site has. Why does your Department not have direct email access as the other departments do?

That hospital in Ajo was closed down many years ago so if there is no central recording office for the State, where are those hospital records archived so that a copy of the original birth certificate can be accessed?1 vote
