  1. Flu vaccine doesn't always work

    Please explain to the public why some of us get the flu even after we have received the flu vaccine. Flu was just confirmed at physician's office. This is of concern since the past 3 years since I moved to Maricopa county, I have received the vaccine each year and each year I get sick with the flu. I doubt i am the only person in this state with this problem

    In 2011, the Arizona State Legislature changed the law, A.R.S. 36-136 (H)(4)(g) and A.R.S. 36-136 (H)(13), to allow residents to produce non-potentially hazardous baked and Confectionery products in their homes and to offer them for commercial sale within the state. The topics listed above provide general guidance for this law and information about required food handling permits in each county. The rules of this program are found in Arizona Administrative Code R-9-8-102.

    I tried to email you about a dirty restaurant but you have not made it easy, it looks like you do not have this ability. May I suggest you make it possible, as for phoning you are closed when I am able to call.

    My name is Jin-young KIM.
    I'm a Korean.
    I am going to study at Arizona State University.
    I have two elementary school students.

    My daughter is 7 years old and 9 years old.

    It is hoped the elementary school on January 07, 2015.

  13. Information on Adverse Health Events?

    Does ADHS collect information on "Never Events" (Otherwise known as "Adverse Health Events")? If so, will this information ever available? Since this type of data tracks very serious errors/issues, it would be great to have access information regarding hospital incidents.

  14. Your SSH certificate expired yesterday

    Please let your tech department know that the SSH certificate is expired. My school nurses cannot get into your vaccination app because of this.

  16. I requested an application for a copy of my birth certificate via the phone & have not received it yet.

    please make it easier to print an application for a copy of a birth certificate. The Microsoft Reader wouldn't open for me.

    Are you looking for general information on immunizations? If you are, try cutting and pasting this site in your browser. If you still have problems, let us know specifically what you are looking for.

  20. It is impossible to follow a Chapter/Article from index to actual reference.

    Try Chapter 10, Article 8. Can you locate Article 8? Or, click on one of the high-lited lines in the cover Index; Your sub-pages [if there is a sub-page] do NOT reference the Chapter ! This is so poorly done, I am in shock. Surely the Az Dept of Health Services can find somebody who can do a better job than this !!!

    Thank you for your comments. The Department is looking into revising the webpages.
    If you need the information, To get to the information now, click on Chapter 10 at the top and that will take you to the Articles that you can click on for the document you are referencing.
    If you still have problems, please call the ADHS main number at ADHS: 602 542-1025.
