  2. I requested an application for a copy of my birth certificate via the phone & have not received it yet.

    please make it easier to print an application for a copy of a birth certificate. The Microsoft Reader wouldn't open for me.

  4. I was given a map that depicts the Brth/Death Department location.

    Invert the picture of the location. It appears to be upside down.

  6. It is impossible to follow a Chapter/Article from index to actual reference.

    Try Chapter 10, Article 8. Can you locate Article 8? Or, click on one of the high-lited lines in the cover Index; Your sub-pages [if there is a sub-page] do NOT reference the Chapter ! This is so poorly done, I am in shock. Surely the Az Dept of Health Services can find somebody who can do a better job than this !!!

    If you need the information, To get to the information now, click on Chapter 10 at the top and that will take you to the Articles that you can click on for the document you are referencing.
