3. "Express Courier" options

   Additional shipping options, shown on your website, are available ONLY w/ UPS. I did not know that, checked USPS and could go no farther.

   It'd be helpful if "UPS" were stated somewhere near that info. VitalChek was unable to help; she said the "only" options were USPS or UPS.

   Once I'd gone back and checked "UPS" instead, I found a small drop down box on a later page that gave me the $17.50 and 19.50 options.

9. I was thrilled that Evusheld got EUA on Dec 8. However, I have been unable to find any info on Evusheld distribution in Arizona.

   Evusheld is a long acting antibody for immune compromised people who are NOT protected by vaccine. A clear plan for Evusheld distribution in Arizona is needed ASAP. Other states have been treating patients since week of Dec 21 --- yet AZ still has NO information or plan. The government has allocated shipments to AZ since Dec 27, but there is still no info on where & how to get it. Plan needs to be shared asap. If there is a barrier to doing this, sharing what is known, what is missing & when we can expect resolution is needed.

10. How do you account for NATURAL immunity? Once you get Covid the antibodies are stronger than any vaccine. I know SEVERAL people that had 2

    I know SO many people that have had 2 shots and STILL got Covid. I. HAD Covid and was in those same circles. I have been fine. I’ve tested every time I travel by plane so I know I’m Covid negative. Where are your numbers for NATURAL IMMUNITY??

11. Why are you letting patients DIE

    If omicron is milder why do you keep saying hospitals are full? Makes no sense.

    And WHERE is the list of facilities that offer monoclonal antibody therapy? Why on earth is Azdh and Gov Ducey not ordering a stockpile? Not everyone has omicron so the monoclonal antibody therapy still works on Delta.

    Why aren't general practioners seeing covid patients? Why are we left in the dark to go home and wait until we can't breathe? My family including disabled and high risk had to self monitor with a pulse oximeter for silent hypoxia at home. It was horrible. When we called our GP they said "what do you want us to do about it ". Nice. Good job azdh.

    Why don't all doctor offices and hospitals let you know if they have monoclonal antibody treatments? Why do doctors working at facilities that don't and won't treat you with the lifesaving therapy tell you to go immediately to a facility that offers it! Why do they continue to treat with failed drugs like remdesivir? (aka run, death is near) What happened to the hippocratic oath? To SAVING LIVES?

    The entire US health gov response to the pandemic has been abysmal. Disgusting.

    The zip code data download for the last 2 days has been incorrect. For example: zip code 85003 is listed today as having 1869 total confirmed cases, but on December 12th the file had 2836 as the total confirmed cases.

    Zip code 85004 something similar: today 747

    Confirmed case count values by zip code on 12/16/2021 are not consistent with preceding values. For example, the 12/16 value for zip 85382 is shown as 4,385. However, the previous day's value was 7,273.

    Displaying COVID-19 cases by Zip code is very helpful, and it would be even more useful if you displayed cases (and deaths) by residence community, e.g. Golden Hills, Aspenwood, Stonegate, etc.

17. Covid data question

    Question only. Since we have started using home Covid 19 test kits, how can the new patient data be correct on your webisite? Most folks do not report the results anywhere so I think we may have more covid in the valley than what is being reported and published. I think the only data that is correct is the number of covid inpatients and ICU . I think maybe a disclaimer would be appropriate when these numbers are being reported to take into consideration the number of unknown positives in home test kits.

    Arizona Genealogy Record Search - cannot open PDF copies of DOB/DOD records to read details - this makes them almost completely useless for genealogy. The icons are grey and not active.....16DEC2021

20. No way to check on Immunization Card Replacement Status

    I submitted my request and all forms on 10/26th. I have not been able to check on status, make sure my request didn't get lost, I email and no one responds. its been over a month. I am over 65 and still can't get my booster shot as my original card had stickers on it and the second dosage fell off. Now I am being penalized by not being able to get a new one. Thank you for all you do but it certainly would help if I could get a status.
