  2. Encourage 4 people per vehicle for vaccinations

    We received our 1st vaccinations Tues and were amazed at how smoothly, professionally, and really pleasantly the whole experience unfolded. But I was sad thinking of all the people trying to get appointments unsuccessfully and it took me many tries. It seems that you could almost double the numbers by allowing the 1st person registered to add 3 others to their registration if all 4 go through in the same vehicle. It would take a minute or 2 more to handle the extra 2 folks at the site and you would double your numbers...satisfy many more and prevent some more…

  3. Give more instructions as to how to sign up for Covid vaccine.

    Have the places where shots will be available closer to where I live in Fountain Hills. My car can't be driven on the freeway or the Beeline Highway; it can't be driven at high speeds; it's too old and has problems. How am I supposed to get to a place that's about 50 miles away? I'm 84 years old and can't afford Uber, Lyft, etc., and there is no public transportation from Fountain Hills!

  4. I can't make a reservation

    I registered my husband for a vaccination appointment but I cannot register myself. I have tried many times. I have tried several different dates, going out February and March, and none of them show any availability!

    your vacine program is a complete joke. 80 years old and can not find an appoint ment in 90 days. from the GOV on down you should all be fired tomorrow.

  7. Register now text is small dark and almost unreadable

    I have 20:15 vision and find the site slow and Register text extremely hard to read. Who is testing the website?

  8. When to arrive for vaccine appt

    I have an appt. for a 6:15pm vaccine at the stadium in Glendale. Do I arrive to get in the long lines at 6:15pm or do I have to arrive in the tent for the vaccine at6:15pm. If so how do I estimate which line to get into and estimate how fast the line is moving?

  9. page match phone instructions

    i thought this said feedback AND SUPPORT but I see no support option & the recorded phone message at the # the portal directs you to call states that I log onto this azdhs.gov to submit employer vaccine request form but I cannot find that option or tab or whatever on the page

  10. when to get in line at stadium for 6:15pm appt.

    Do I get in line for my 6:15pm appt. at 6:15 or do I have to in some way time the waiting line to arrive in the tent at 6:15pm?

  15. That the Kordinator web site to register for the Covid vaccine would work! Hard to get an appointment when the web site doesn’t work!

    Your pharmacy site for 75 and older isn’t up and running. Why? Pharmacy said they have vaccine but need appointments made on line, yet on line site doesn’t take any appointments! HELP!! Make it work!!!

  16. Way to confirm COVID-19 vaccine appointment

    Both my wife and I signed up for COVID vaccine on 1/11/2021. There seems to be no way to confirm our appointments. We did not receive any email confirmation, Was the website not working correctly.

  17. Improve this site!

    I just got hung at a point in the vaccination registration program. I took a screen shot, but there is no "attachment" option on this feedback form. Why not? Had several other very confusing points with this software. There should always be a "Site Manager Feedback" option on EVERY page.

  20. The hospital bed and ICU bed availability are percentages of what? (What’s the denominator?)

    Please provide definitions of the terms ADHS uses on the hospital and ICU bed availability dashboard report. This includes defining the terms in the denominator of these percentage calculations. Please send this information to my email mueller.jeff@mayo.edu .
