  2. Use hydroxychloroquin for all suspected covid-19 cases for 7 days

    A. 7 days for all persons immediately who show symptoms
    B. 7 days for all who test positive
    C. I suspect (only from info on internet) that most of the serious problems stem from either blood clots or a hyperactive immune response

  3. For COVID 19 tests 1B show the available dates and times

    You need to show what dates and times are available. It is impossible to schedule without knowing that.

  4. Better scheduling information for Covid Scheduling.

    Why doesnt your web site let you select a date and time for a Covid Vaccination? And why does it not give you a message when and why you cant ?

    It just leaves you hanging in limbo with nobody to call for help?

  5. THIS SITE IS IMPOSSIBLE TO USE. I HAVE TRIED FOR 5 DAYS TO MAKE AN APPOINTMENT. THE SITE IS IMPOSSIBLE TO GET THROUGH UNLESS YOU ARE 25.

    Redo the web site so that it flows and can be seen on the average laptop screen.. The next button is way to far away from the information requested... The idea of a calendar button to get the date is not intuitive for those not used to using one. I for one had never seen that, only type in the date. Took me many tries to find out how to use it... I only have a Dr. education, not a Dr. in computer science. I certainly am not the only one that has had a problem with trying to get…

  6. Change your reporting matrix of COVID cases to over 5,000

    Almost the entire map of AZ is red now which depicts over 1,000 cases by zip code. This is no longer helpful. The matrix should be updated to reflect numbers greater than 1,000 to get a more meaningful picture of communities who have more cases.

  7. Inability to schedulde appointment for covid-19 vaccine

    I am 75 years old, have registered on the AZ covid-19 website, but am unable to schedule an appointment to receive the vaccine. For every date between 1/16/21 thru 2/5/21, I receive the message "There are currently no events available..." Why are no vaccination sites available within 50 miles of my home in Sierra Vista for the next 3 weeks? I am especially concerned because next Tuesday the state is making the vaccine available to even more people, and I may be forced to wait months to find an available appointment.

  8. Show available dates and times

    COVID 19 Vaccination Registration
    This is nuts, why when I try to register for a vaccination why doesn't it show me a list of available dates and times versus me having to pick a date click 17 times to only find out there is nothing available and then to start that all over with a new date. Highly recommend showing a a list of available dates and times for folks to select from. The existing method is time consuming and frustrating.

  9. 2nd covid vaccine availability to sign up

    I have an appointment for myself and family on the 21st of January at Chandler. When will appointments open for my 2nd vaccine? Please email me with that information. I have tried to sign up on the website for the second vaccine and it says no appointments are available.

  10. I think your podvaccine scheduling programming sucks.

    1. Instead of asking when you would like to be vaccinated, present slots that are available and let the customer pick one.

    2. The question "Do you have insurance?" in unanswerable for us on Medicare ADVANTAGE. (a 3rd option).

  11. Allow rural people to get the Covid vaccination

    Your current portal to sign up for Covid vaccinations absolutely prevents anyone living further than 50 miles from a site with vaccine from signing up. This includes people in the highest risk categories. Is the plan to kill rural Arizonans? Why are you doing this?

  12. Bring back previous genealogy.az.gov website!

    I have been using genealogy.az.gov for years. The new Genealogy Record Search on azdhs.gov is awful! The old website displayed far more information, including: more records per page, total of how many records found, which county the record was from, and a color distinction between death and birth records. Also, the new webpage is far slower at searching the records and to make matters more frustrating, it doesn't even produce the same records. Also, all the old links that were saved to the records are now obsolete. At least have the old pdf pages link to same record, but at…

  13. Recent changes to the online search of historic vital records is a terrible mistake

    You have made the search less effective, and no longer searchable by county, and date ranges. Please restore the old functionality.

  15. Appointment page for vaccine is not working

    My 78 year old mother is not getting an appointment for Covid19 vaccine,
    my father passed away in December from Covid19

    On the page to choose a date, you need a "Soonest available" checkmark to bypass the guessing game.

    Also, on the insurance page, the dropdown to choose "self" or a dependent isn't working.

  17. There should be an easier to make an appointment to get the Covid vaccine. I have been trying for 2 days and the site won't let me registe

    Try to make the form clearer to understand.

    Allow a user to check the boxes that are required.

  19. Your Vaccine Registration Process is Non-Functional

    I am trying to make your system work more easily and not jam up with traffic: Your logic should be: STEP 1: Applicant enters all data required and it is saved (presently is erased and must be re-entered on each search). STEP 2: Rather than ask persons for a date to search vaccine event availability, the site should suggest the next available date and location to the applicant (!) I have been searching dates endlessly. I hit a wall (no available dates) then must leave, lose all data, and must start again (entering all data and searching for dates again!).…

  20. Frustration signing up for vaccination

    Why cant you show first available slot by location instead of having to go day by day only to find no slots. I tried 3 locations into February with no luck after spending 45 minutes
