Redo the web site so that it flows and can be seen on the average laptop screen.. The next button is way to far away from the information requested... The idea of a calendar button to get the date is not intuitive for those not used to using one. I for one had never seen that, only type in the date. Took me many tries to find out how to use it... I only have a Dr. education, not a Dr. in computer science. I certainly am not the only one that has had a problem with trying to get an appointment.
