  1. Trauma Surgical Facilities

    In recent years, Account Pamela & Associates conducted a feasibility study on Trauma Surgical Facilities in Arizona. More recent research reveal the southern areas of Phoenix are without adequate medical facilities and hospitals. Specifically, the Ahwatukee and South Mountain area is currently observed for Trauma Surgical Facilities. FYI: The nearest hospital in some locations are between 10 and 17 miles away. Account Pamela & Associates in coming months will propose Trauma Surgical Facilities and other Life Care Facilities for the growing areas of Arizona.

  3. Your site states that my cookies are disabled. They are NOT yet your message box will not send with the continuous rejection message that co

    our contact section refuses to send my message stating that cookies are disabled. That is not so. Cookies are enabled so your tech needs to get to work and fix whatever problem your site has. Why does your Department not have direct email access as the other departments do?
    Too busy to answer email? too much work? Public servants, Bah!

    My wife, Francisca Amada Acuna was born in the Ajo Hospital on 3 August, 1965. Would that birth have been automatically registered at your central records office for births, deaths and marriages? If so, how do we apply for a copy of the birth record, please?
    That hospital in Ajo was closed down many years ago so if there is no central recording office for the State, where are those hospital records archived so that a copy of the original birth certificate can be accessed?

  5. Medical Marijuana CME

    I was looking for the online medical marijuana CME module. I went to azdhs.gov/cme, and it presents the option of "medical marijuana continuing medical education (CME) module." When I click on the link, it just sends me to a frequently asked questions page. Do you have a proper link to the module?

  8. quick link for providers to log in

    I am having difficulty finding the log in page for the DAVE site to be able to fill out a death certificate. once on the web page, it is not intuitive to be able to find the log in.

  13. Error CORRECTIONS

    I am an almost daily user of your data base, but today I came across a mess. All entries for Pima County, 1912 Deaths, numbers 33 through 48 are incorrect names and they are from Maricopa and Mohave counties. Are there coresponding entries elsewhere that are correct for Pima County?
    As an aside, when searching for any given year in Pima County (and I assume each county has similiar entries), there are about 30 inirial entries that are the same for any given year search that have incorrect dates which anyone with access to your system could correct in a…

  14. Convert the BRFSS survey to digital please!

    I participated in the BRFSS by phone today and found the process to be almost unbearably painful. As a health care provider I appreciate the value of the data collected, but doing it as a scripted phone interview was overly long and difficult. I was able to find and read the questions on-line and should be able to complete the task more accurately and in a fraction of the time electronically. Please consider offering an electronic survey link in the future. I'm also curious why the most recent report available is from 2015? My interviewer had below average English speaking…

  20. Fraudulent Billing Information

    When I submitted my insurance information for my newborn's screening test, I received a bill from ADHS saying that the patient could not be identified by the insurance as insured. I checked with my third-party administrator and found out that the claim had been accepted and was being processed. When I called your billing department to find out why that note had been added, the only response I got was that my third-party administrator "takes a long time" and that a note would be added to my account, after which I was hung up on. No mention was made of…
