  1. How can I verify cards older than Feb 2017?

    I have authorization as law enforcement to verify a card. The current one I am checking pre-dates the changes in Feb 2017, so there are only 19 characters on the card, not 20+. The system is not allowing for verification of the number because it is too short.

  2. Public smoking

    The government could make so much money if you implemented a no public smoking law. Each person that gets caught smoking in public would be a $1,000 fine for first offense. Only places people should be smoking is in their private homes. Smoking near other people with the second hand smoke is giving everyone health issues. Plus, doing this could save millions of lives and give the government more income to fix roads and things that need to be done.   3. filed a complaint in 02/17 regarding Phoenix Baptist Hospital negative treatment, and civil rights violation, of patient privacy.

    been several months and have not heard a response from the public health regarding this matter i am curious why know one has contacted me with the status of this investigation.   5. Cell phone application for Diabetes Management

    To Whom It May Concern;

                                      I am writing you concerning a wonderful, easy, effective and user

    friendly I-Phone application for type 1/type 2 Diabetics and persons above their BMI

    for age/*** wanting to reduce their chance of developing a metabolically based disease.

    The phone application guides patients to select better foods based upon their G.I or G.L,

    (Glycemic Index or Glycemic Load). The application however is very effective because the

    foods are grouped by similar "FLAVOR". Hence the accomplished goal of Lowering the A1C to 6

  10. Fair - feel there should be some grace shown for change in renewal policy -late fee very high!

    Also - online not user friendly in that additional support documents must be faxed or emailed, due to size

  14. Why are we not appropriating the money afforded the mental health population?

    I was told by the news that more funds would be appropriated to mental health. Where are these funds and when will they benefit Arizona mental health patients? I am SMI (Seriously Mentally Ill) with Bipolar. I take my medications properly and never miss an appointment to my NP (nurse practitioner) and still I have had one of my providers leave because of better opportunities and one laid off for what I can only assume is financial reasons. I do not do well when my doctor or NP in my case is changed. Why do they call for more money…

  15. Emailing the renewal reminder

    Is not working. It got put to spam, and the thing expired. Lost a caregiver card that was just "purchased" at the tune of $200 in August. So here we are. Suggestion - US mail the reminder, instead of emailing.

    Hi, I'm writing a grant and as usual, I end up here: http://azdhs.gov/prevention/health-systems-development/data-reports-maps/index.php
    Why? Because it's the only resource for demographic data about RURAL communities in Arizona. I notice that you have made some changes to your website to make the PCA community profiles much easier to find, esp if one is looking for info on a variety of RURAL communities. Since our service area is all of southeast Arizona, this is a wonderful resource, since trying to find demographic information about rural places on the US Census website is like the proverbial needle in the haystack.
