  2. Why full price of $ 300 for renewal with incurable diseases

    Don t punish patients with expiring card and an ongoing incurable disease like Parkinson's with a $ 150 fee from a doctor who is not even the patients physician. He has also developed another cancer for which he is paying $ 2000 for the radiation. the products in dispensaries are expensive enough. Why can his physician/oncologist NOT write up the health condition and reason for a medical marijuana card yo avoid that fee? It is really unfair, just an additional income for doctors.     I am from the north East amd bought my son to Mohave Mental health for treatment of his heroin addiction. Not only did he have to wait two months to get suboxone prescribed. He finally got the prescription and was told not to take it until the following week we he went to his next doctors appointment, and to drive in there in withdrawls to take it in fron of the doctor. he sais what am i supposed to do over the next 5 days, continue to use heroin.   7. Cannot FIND WAYS to select/specify a designated CARE GIVER

    I live in Kearny, Arizona. Superior Organics Dispensary CLOSED. I would like to select a DESIGNATED CAREGIVER as a supplier. But I CANNOT FIND THE APPLICATION FORM that I need to fill out to identify and select my Designated Care Giver. Can you help?

    I cannot even contact you to ask a question. I am sure this is deliberate in nature. All I want to know is why you announce on your site that the card is $75 to $150 and yet I have been informed that it is $300. That amount demonstrates the corruption in the state's government. You advertise one thing but the reality is something else. You people are horrible.

  14. Hire a completely new staff

    I can get better customer service from a lobster in the middle of the Atlantic ocean in the pit of a whale shark's stomach that has croaked over and been eaten by 19 great white sharks. Your customer services are worse than a banana peel that has been sitting in the garbage can in my bathroom for the last 7 months. FIX IT

  17. Smoking Violations Submittal - does not confirm success

    Your Android and regular computer submission process does not confirm whether or not submission was successfully submitted - I tried numerous times and on multiple applications to submit my concern/complaint - could you please confirm that my concerns were received?

  19. complaint form suggestions

    Hi:
    1. your complaint form should issue a email confirmation it was received complete with complaint number, copy of message sent, and information as to what happens next.
    2. your complaint form should have a way to attach documents
    3. this service should include updates so you can view the progress of the complaint.
