  1. Website Suggestion

    I have a suggestion for a website on your Injury Prevention Resources page. It is the Kids Safety Network (www.kidssafetynetwork.com). They have a lot of great resources on injury prevention, health & product recalls. I just thought it would be a great fit for your website. Thanks!

  3. I requested an application for a copy of my birth certificate via the phone & have not received it yet.

    please make it easier to print an application for a copy of a birth certificate. The Microsoft Reader wouldn't open for me.

  7. I was given a map that depicts the Brth/Death Department location.

    Invert the picture of the location. It appears to be upside down.

  8. It is impossible to follow a Chapter/Article from index to actual reference.

    Try Chapter 10, Article 8. Can you locate Article 8? Or, click on one of the high-lited lines in the cover Index; Your sub-pages [if there is a sub-page] do NOT reference the Chapter ! This is so poorly done, I am in shock. Surely the Az Dept of Health Services can find somebody who can do a better job than this !!!

    Thank you for your comments. The Department is looking into revising the webpages.
    If you need the information, To get to the information now, click on Chapter 10 at the top and that will take you to the Articles that you can click on for the document you are referencing.

  14. I tried to sign up for account for vaccine registration

    Unable to set up account to register for vaccine. I gave correct email address and was told I would recieve email with link to set up password. Never got email. When I tried to register again I was told there was an account under that email.

  15. Covid Vaccination Schedule Site

    The acct database does NOT store personal info, responses to questions, insurance info, etc leading up to scheduling. That is a ridiculous waste of time.

    Why can't we create multiple family member profiles and schedule appts together????

    Why can't I make an appt for a "Transportation" worker (which is a current priority group) when that's the sole criteria response? There are no appts slots available?

    When I selected an available schedule slot and proceeded to confirm, the slot was then gone. I had to start over. WHY???

  18. Covid Vaccination Schedule Browser Issue

    I believe the Covid Vaccination page indicated that Chrome, Firefox and Safari browsers were all supported. FYI. Safari is NOT stable. I could not create an acct with it as I couldn't get past the Date of Birth/Phone # fields. The page kept asking for correctly formatted info. The Birth Date field did not have a format guide. I had to use Chrome to create the acct.

    Then, I logged into my acct with Safari and after I filled out all the info, I got stuck at the scheduling page with a spinning progress graphic.
