Soon after Covid-19 shots are causing heart attacks
I feel this is being overlooked.
I am a male age 68,in good health, and no heart issues.
11 days after rec my Covid-19 shot, I had a heart attack, and one stent was placed.
My wife was on vacation last week, and talked to a lady nurse from Penn. The nurse told my wife, that 7 days after she had a Covid-19 shot,
had a heart attack, with complications, and was in the hospital for 6 months.
I am on Medicare, thank God,but on heart pills for the rest of my life.
What if I had no insurance, and would probably be homeless !

Emendation for ADHS Licensing Complaint ID 2021-LTC9971:
THIS JUST IN!!—the manager for Precious Heart Assisted Living had a small cache of colostomy bags kept in reserve so that she could dole them out as needed, so the issues referenced in Complaint ID 2021-LTC9971 are now pretty much in the clear. As the report implies (though probably not all that clearly), it was never my intent to charge the facility with any wrongdoing—I feel that the manager and her attendant did the best they could under the circumstances, even recommending I get the contact information for my medical insurance to make sure I'm not trying to kill the…

Az Programs.
Stop needing to test flower. Since the rec law has passed , my prices have shy rocketed ,and the quality sucks. More importantly is the need for deli style service.
All the concentrated are legal. edibles can be unpredictable at best. Yet you wan the flower tested. Makes zero sense. I want to be able to speak to a bud tender and see what I am getting. I am getting ripped off1 vote -
Participation - Mental Wellbeing Taskforce
My name is Deb Jorgensen and as the OIFA Administrator for Care1st I cover Northern AZ I would be grateful if allowed to participate on the Taskforce. Isolation in NAZ is a whole other animal; resources are not as plentiful and access/distance is always a factor - on the flip side there is often more sense of community in our small towns. my work cell is 480.205.2305 and my email address is djorgens@care1staz.com. As someone(a peer) who is usually in the community for my position, and for personal wellbeing, I can also speak to this extended period of loneliness/isolation…

I would like to be able to email a question of how to get my COVID 19 vaccination card replaced. It was destroyed in the wash and with the new government mandates I will need a replacement.

Could you provide an estimate for the COVID dashboard system 8/24 update completion?
The system update was supposed to be completed by 8:00 PM on 8/24. As of now (8/25, 8:40 PM) COVID dashboard is still not working. Could you post an expected completion time frame for this?

Bring back previous genealogy.az.gov website!
I have been using genealogy.az.gov for years. The new Genealogy Record Search on azdhs.gov is awful! The old website displayed far more information, including: more records per page, total of how many records found, which county the record was from, and a color distinction between death and birth records. Also, the new webpage is far slower at searching the records and to make matters more frustrating, it doesn't even produce the same records. Also, all the old links that were saved to the records are now obsolete. At least have the old pdf pages link to same record, but at…

The new birth and death certificate search engine is a total disaster.
Returning the old vital records search engine would solve the problem...

Please include number of RECOVERED COVID19 patients
Please include number of RECOVERED COVID19 patients. The current numbers let us know how bad it is... it would be very helpful to also see something hopeful.

Recent changes to the online search of historic vital records is a terrible mistake
You have made the search less effective, and no longer searchable by county, and date ranges. Please restore the old functionality.

Number of Hospitalizations and Deaths by County
I would really like to know why you cannot publish the Covid-19 Hospitalization and Death numbers by county. The head of statistics at the BBC published an article on 7/27/2021 entitled “Coronavirus: Four numbers that reveal the real trends” and the very first one was “The number of new hospital admissions”
He says that:
“Daily case numbers get the most attention because they're the first to arrive. But they're flaky. They depend on things that tell us nothing about the virus, like whether people decide to get tested or how quickly test results come back. So scientists use other measures…1 vote -
The download of vaccinations by zip is not working.
On Jul 25, 2021 I tried for the first time to download vaccinations by zip, but I got no file, no error message, no progress indicator in my browser. By the way, I have successfully downloaded cases by zip for over a year. It often had this bug and others, but they were fixed. Does vaccination download by zip work? I will keep trying to see if today was a "fluke."

"Confirmed Cases" should represent individuals infected, not simply number of positive test results.
"Confirmed Cases" is confusing since it appears to be reported by the test specimen instead of individuals infected. I don't know for sure if this is what's going on, but it seems like it. If so, AZDHS ought to correspond test results with individuals tested to account for repeat testing, and report a proper "confirmed cases" count. This will provide a more accurate picture of Arizona's CV19 situation, and thus, better policy decisions. Points addressed with slightly more detail in this AZ Daily Star letter and the first comment on it: https://tucson.com/opinion/letters/local-issues/letter-covid-19-data-in-arizona-garbage-in-garbage-out/article_56f828b8-c140-11ea-a697-b3dbebea2bee.html?mode=comments
