Executive Order 2020-43 #5 refers to a form that may be issued by ADHS to reopen, where is this form available?

Contact tracing
I would like to see coronavirus covid-19 contact categories:
e.g. nursing home resident, nursing care employment, foreign travel, domestic travel, airline travel, work related, large group gathering outdoors, large group gathering indoors, shopping, family gathering. This would give people a better idea of situations to avoid.1 vote -
Garbage In Garbage Out
The statistics are worthless with your methodology. If we can't differentiate between even suspected and confirmed cases and won't differentiate between those admitted 'for' Covid and those admitted 'with' Covid as the most basic of metrics the information is useless for decision making. I'm left to believe leadership is incompetent.

Better data correlation
On the COVID data, could we have better correlation of the data? Having different numbers for the same metric is frustrating. For example, on the ‘COVID 19 Deaths’ tab, the box at the top indicates 44 Deaths reported today but the chart below indicates only one. If the number in the box is a running total, it should be labeled as such. Likewise under ‘Hospitalizations’ it says there are 4736 hospitalized COVID cases but under the ‘Hospital COVID-19 Specific Metrics’ tab the number is 2793. Which is it?

that it the AZDHS series of websites are very difficult to navigate. It needs an index.
The DHS site complex needs an index. It is extremely difficult to find COVID19 information without already knowing which document you want to find, e.g. I wanted to see how to get to this page ( https://www.azdhs.gov/documents/preparedness/epidemiology-disease-control/infectious-disease-epidemiology/novel-coronavirus/sdmac/covid-19-addendum.pdf ) which I saw posted on Facebook, but found NO path to get there when starting from the homepage without searching for words in the document. Previous searches for specific COVID19 policies and information were also frustrating.

Covid numbers - tested in Spanish database are incorrectly labeled - it should read total numbers - not total positive.
your tables in spanish are translated incorrectly. please see that the total number of test reported for pcr and serology are not all positive but just totals. it is giving the wrong impression to Spanish speaking audience.

COVID-19 Addendum (06/12/2020) has a typo in its examples in Summary Table 1
The addendum suggests the following example to determine a Triage Priority Score: "Example: SOFA SCORE 6(2 points) + DEATH EXPECTED WITHIN 5 YRS DESPITE SUCCESSFUL TREATMENT OF ACUTE ILLNESS(4 points)= TRIAGE PRIORITY SCORE:6."
According to the table above this example, the "Death expected within 5 years despite successful treatment of acute illness" condition adds 2 points (not 4), so the Triage Priority Score for this example would be 4, not 6.

Please provide an additional Covid 19 data point
Please share #s or % of Covid 19 cases needing hospitalization and especially ICU. More than infection rates, these are critical data for the public to understand severity and impact to medical services. The ICU beds used and available, and C19 specific PPE, and ventilators are the limiting factors that we want insight into.
Thanks for all the information that you have been pulling together and making available! it’s definitely a challenge with such wide-ranging and variety of sources, and no methodological standards for which data points are tracked or timing for compilation!5 votes -
Update the COVID Data Dashboard to Accurate Percentage
Update the data dashboard numbers to be accurate. If you've tested 88,000 people and there's over 9000 positive cases, that's more than an 8% total of the population. Someone doesn't know how to do math.

Please adjust scale in zip code map again
40 as the highest level means zips with 41 cases appear the same as those with >100. Can you please adjust the scale?

We should be trying to find the asymptomatic people to stop the spread!!!
Allow a certain number of people each week to be tested who do not qualify according to your rules. I have travelled across country by car to attend a funeral and been thru places where thousands of people have visited (truck stops, etc...). I would like to get tested but cannot.

The Deaths category only shows Maricopa county numbers, no matter which county is clicked on
The program just needs to be tweaked. It used to work...

Improve testing accessibility
My friend has been exposed to,COVID 19 and needs a test. She does not need to sit in a hot car for 12 hours waiting. She has been on hold for over an hour with the banner test line. I know we are old and according to Current thinking expendable but her experience is no doubt shared by those you want to save. Get it together. This is not acceptable. Hire more customer service representatives or contract with additional providers. Thank you for your time and attention

Your data is invalid. Please either take down the site or fix it. There's plenty of reliable sites tracking your data and doing a FAR better job.

Why is nothing being said about wearing masks in public? I only see it from a month ago, and even then, it was just a suggestion. If AZ wants us to wear masks around other people, then it should be stated as such in the updated orders -- not just to practice social distancing measures.
