Why is there a difference between what the percent stated by Dr. Christ on 10-22-20 of 3.9 & 5.5 does match what’s on the dashboard?

Improvement to covid registry form
You should indicate the date of birth format. Apparently it is YYYY/MM/DD; but you don't say so.
The field for the telephone number isn't wide enough.
You say a code has been sent, but you don't indicate where to.
Email or telephone? You should say.
The system does not send a code to ether. The first form doesn't work well in chrome, but it does in firefox (I have both, so I know)1 vote -
Print on your email documents is too small for me to read.
Use larger fonts for emailed documents,please.

Where can I find daily update on the number of vaccines administered?
I would like to see on the Covid dashboard the number of vaccines administered by county. I would like it to be updated daily.

A link or page for people who's vaccine invite link isn't working properly
How about some info for those of us in healthcare who are trying to activate the "Vaccine Event Invitation" email link? The "Secondary Code" is not showing up in my inbox so I can't proceed, and I just heard on KJZZ that you're aware of this as a problem w/ many people, so how about a solution?

Vaccinate people with underlying medical conditions
Patients with compromised immune deficiency and eosinophilic uncontrolled asthma should receive vaccine as soon as patients in nursing homes receive theirs. I get IVIG monthly (may prevent/lesson severity) of infections. I also get a monthly biologic prescription injection to moderate white blood cells. I am at very high risk for covid and need early vaccination. Can you arrange this? Neither my primary care physician, immunologist, or pulmonologist know the vaccination protocol to vet patients and schedule vaccinations in Tucson.

Early covid vaccination for immune-comprised patients (before general population)
I'm requesting early covid vaccination because I have CVID (immune disorder that requires monthly IgG infusions, and also eosinophilic asthma for uncontrolled asthma for which I get a biologic medical injection monthly. Very high risk underlying conditions. Requesting early vaccination please.

Group 3: vet and administer covid vaccination early to patients (not to general population) with underlying serious medical conditions. I have CVID (immune disorder) and eosinophilic asthma, both requiring IVIG and biologic injection monthly. I am high risk, requesting early vaccination.

This page says that the data were developed by the Office of Employment and Population Statistics within the Arizona Department of Administration but I can't find that office and I would like to know more about how they created the population denominator estimates.

prisoners get vaccination?
I DO NOT think prisoners should be given a vaccination before the tax paying public that pays for them.High risk or not.A child *****,killer,*** should get a vaccination before good honest hard working tax payers???? Save the child ******** and roll the dice with people that pay for them?Whats wrong with America for even considering that?

Seeing other patient's info on my file
Dude, you have big problems with site today. Was fine at 1am...could enter my info and my phone number without issue. Decided to wait to sign up for vaccine until today.
Now, at 9:20am, have to use arrow keys to enter phone # because cursor moves to the 'left' of the first entered number instead of the 'right'.Next, and the biggest issue, I used the 'previous' button and it took me back to the name/address page and someone else's info was there in the name field. so my address was there, but someone named Monica something or other and…

Extend driver's licenses expiring in early 2021
You say we are safer at home, but Gov Ducey’s executive order extending driver’s licenses expires on Dec 31. Thus licenses expiring in January, as mine will, will in fact expire unless we leave our homes and expose ourselves and DMV staff to infection. ADOT says it is exploring “options” but it has been saying that since October and nothing has been done. Without a driver’s license it will become impossible to get food or go to doctor’s appointments except with an Uber taxi which means even more exposure to possible virus. This is really urgent.

Please use a list option as well as a map. For elderly people its difficult for them to manage the zooming option on the map.
the map isn't helpful even for people that know how to use the internet. In times of stress you need a list.

Add number and location/age vaccinations
It’s time to add information on vaccinations onto your data dashboard. How about telling us who has been vaccinated and where , their age , where they live , and other information and we need to know to see how Arizona‘s vaccination program is going.

Place those 75 and over in phase 1b of the Covid vaccination schedule.
CDC has recommended that those people age 75 and older be placed in phase 1b of the vaccination schedule. Right now, in the AZ plan, they are in 1c. I suggest that they be moved earlier, since they have a 30x risk of death from Covid.

Coronavirus Vaccine tracker
Please provide updated data on number of vaccine shots given, plus number of people innoculated (after 2nd dose), and percent of population that has been vaccinated.
