  1. Redirect broken with HTTPS

    AZHealth dot gov appears to be used to serve redirects to the AZDHS website. However, if I visit an AZHealth link with an HTTPS header, the website tries to serve a page with the wrong SSL certificate. I suggest that the HTTPS handler needs to be updated (especially considering that dot gov TLD will be moving to pre-load HSTS). Specifically, I observed this behavior with a link from a PODVaccine email newsletter, pointing to https://azhealth.gov/findvaccine. Thanks!

  2. Your Vaccine Registration Process is Non-Functional

    I am trying to make your system work more easily and not jam up with traffic: Your logic should be: STEP 1: Applicant enters all data required and it is saved (presently is erased and must be re-entered on each search). STEP 2: Rather than ask persons for a date to search vaccine event availability, the site should suggest the next available date and location to the applicant (!) I have been searching dates endlessly. I hit a wall (no available dates) then must leave, lose all data, and must start again (entering all data and searching for dates again!).…

  3. That the Kordinator web site to register for the Covid vaccine would work! Hard to get an appointment when the web site doesn’t work!

    Your pharmacy site for 75 and older isn’t up and running. Why? Pharmacy said they have vaccine but need appointments made on line, yet on line site doesn’t take any appointments! HELP!! Make it work!!!

  4. Way to confirm COVID-19 vaccine appointment

    Both my wife and I signed up for COVID vaccine on 1/11/2021. There seems to be no way to confirm our appointments. We did not receive any email confirmation, Was the website not working correctly.

  5. Improve this site!

    I just got hung at a point in the vaccination registration program. I took a screen shot, but there is no "attachment" option on this feedback form. Why not? Had several other very confusing points with this software. There should always be a "Site Manager Feedback" option on EVERY page.

  6. 75 don’t drive a lot

    Please offer vaccine sit on east side of Pheonix for seniors. The website says pharmacies are available but can’t make an appointment. Call to pharmacy said they have vaccine but must make an appointment on web site . Have tried many times for 2 days, site not working!

    Make this easier for the elderly and if there are no slots it would be good to know before spending an hour putting in information.

  9. Negative As I Cannot Register for Covid Vacination

    Went to create account so that I can schedule an appoint for Covid Vaccine but no verification code is ever sent. I have been trying for 2 days and nothing. Very poor site with no one to contact.

  11. show available time slot for vaccine appointment without signing in

    It will be great to provide an almost up-to-date view of the available appointment without signing-in by category (phase 1a, 1b, second dose) .This will encourage people to utilize any available time slot without checking every possible date and location

  12. Make COVID-19 vaccine scheduler work!

    Why was this only developed on Google Chrome - why is the page not found when I attempt to put my elderly mother's info in to schedule her for a vaccine? This should have been in development months ago - it's egregious that this can't be done on a mobile device, can't use a Cox email. Whoever is the IT genius behind this scheduling platform needs to be evaluated because it's not working.

  13. Your web page for COVID vaccination signup

    To say the least, this page doesn't reflect well on your group, AZDHS or the State. Among other things the procedure for entering a birth date is defective - click on the calendar for a list of years and you get years starting with 1 (presumably AD). I finally scrolled forward 1900+ years to get to 1945. It is impossible to directly enter the year. After we got past this your code apparently didn't like Windows 10/Chrome, giving an error message "tablets not supported."

    You have had three months to do this! This page looks like it was coded by…

  14. Have a secure site for pre-registration for vaccines and have a phone number

    My browser will not allow access to your insecure site and there is no phone number to call for the State Farm Stadium site. So, how is one supposed to register?

  18. Kudos for excellent Covid Vaccine Drive-Through Site at the U of A

    I received my Covid vaccine this afternoon at the U of A drive-through site and was amazed and impressed at the efficiency, thoroughness & friendliness of the whole experience. Bravo to all for job done exceptionally well - Thanks!

  19. Number of deaths chart is not consistent with daily deaths

    Yesterday (April 28th)18 new deaths were reported. Today the number in the graph shows only 2 for April 28th. Why the discrepancy?

    It would be best to have a 'next available' option in the scheduler therefore not having to toggle through each date individually
