Can you check Madison street jail for funguses or any type diseases growing on the floors?
There are many people I know that have came up with many different types of foot fungus and also a variety of diseases of many sorts. Some of them include Hepatitis and also MRSA. Can we have the floors and even the water tested for diseases. I only came up with a terrible foot fungus or athletes foot for at least 9 months. A concerned and innocent person that had to stay there for a night or so. I was innocent but later agreed to stupid charges. Can we have to closed down jail tested. I think it would be…

my wife has dui. we cannot figure out how to get list of approved alcohol counseling agencies on line. just want a list...cannot figure website out. we are old and do not understand computers very well.

3rd party vitalchek
Ordered birth certificate on the 17th of August. Was told to keep checking the mail. It was sent to you on the 24th of August. Today is the 19th of Sept. AZDHS website states for faster service we use 3rd party services. It is so inconvenient and such a headache. I get nowhere talking with anyone. Told to go to the website and click on the big red link and I would be able to request another copy for free of charge. To no surprise, the link went to vitalchek so I can order a new one for a fee.…

problems with expedited service for birth certificates
I went thru Vital Chek for expedited service, nothing but a joke, they kept saying they needed more proof of ID I supplied them with (BC, drivers license, and military ID), I never received my certificate for my son but they sure enough charged my card for the service, WHAT A JOKE

I am being ignored by the City of Sedona and Coccino County
I have been suffering headaches and nasal congestion for months because Cocconino County will not put out fires!! It is said that the fires cannot be reached, but no airplane water release has been used. California would not allow a fire to just simmer for weeks on end!! Sedona residents have suffered from smoke related problems for most of the summer continuing into the fall with no end in sight. Health issues are being ignored by local government! Can I sue the state for such neglect?

I would like to suggest that the AZDHS stop passing regulations that leave my wife in agonizing pain!
My idea is that our government try an intelligent way to stop the "opioid epidemic." I understand that you absolutely do not care about people. You're here for your own agenda. Everyone with a brain in their head already knows that.
I would suggest, however, that if you don't want more dissent, more anti-government anger, and more homeland terrorists, you make an effort to stop ******* off your constituents. Sitting in a hospital where doctors refuse to prescribe proper pain medication, watching my wife writhe in agony and ***, because you idiots can't regulate your government effectively, simply infuriates me.

Please make it easier for physicians to find the log in to register a death certificate. I have to search too long to find it

My experience with the Arizona Office of Vital Statistics has been terrible. I am an attorney in Michigan who has been trying for 3 weeks
to get a death certificate for an Arizona decedent. I am aware that Arizona is a closed records state. I reviewed Arizona law and administrative rules before sending a detailed written request setting forth my clients’ eligibilty (to clear title to jointly held property in Michigan). After 3 weeks I got a “form” deficiency letter that was not specific to my detailed request. A letter faxed to the supervisor went unanswered. Please help me. Randy Martinek, Esq. Cell: 810-434-3743. as stated above. I cannot find the info that you state elsewhere is public. I tried to submit online form emails twice on those sites, and they both failed for no apparent reason - the error said something like there was an invalid character in the subject line, of which there is none. No subject line. Can't correct what isn't there (or visible to me, anyway). where do I get verification of a doctor's license to practice in AZ?? thank you.
