Add an event date input by user. Use the old program as a model, it worked very well!
The old search engine worked just fine. Why on earth did you gut the old program for this monstrosity??? Show deaths and births by Surname, Date and County,

Post a location for each area of the county and list groups eligible for the vaccine with the next date and times for appt's
I've been trying for weeks to get an app't for Covid 19 vaccine. My wife is 79 & I'm 85. We live in the east valley, Mesa. Every time it always ends that every appointment is taken for at least 2 months.....help please!

Totally unreliable links -- all to unavailable vaccine locations
It clearly would be easier to allow people to get in a queue and then notify them when the $%^&* vaccine becomes available near their address. This current system is worse than looking for a needle in a haystack. So poorly managed; I expect to get vaccine in June and I'm over 65. Come to AZ to DIE. Pima County is inept.

ideas for not wasting vaccine
1) public service emergency announcements and 2) standby areas for 100 or so cars like at an airport
Wasting vaccine is criminal. I help seniors enroll who have trouble with computers. You need more ombudsman to help with them on the phones.1 vote -

Provide a way to call or find out if your appointment went through.

vaccine finder map
The map is incredibly frustrating, at least for Maricopa County. Is there SOME REASON why you can't highlight sites where the vaccine is actually available so I'm not going from place to place only to learn that it's NOT available. Probably good PR to have a lot of dots on that map but really incompetent to just list them when they are not really taking appointments. Are vaccines in Maricopa Country available at Frys, Walgreen's, CVS, etc.? If not, WHY are they listed on the vaccine finder map.

When people don’t receive an invitation for there second prized shot, give them a way to contact you.
Include “no invitation sent to you” as a choice to check on the registration page of the signing up for a COVID vaccination.

Excellent Job at State Farm!
We drove to Phoenix yesterday to get our first dose of the Pfizer COVID vaccine. We went to the state run site at State Farm Stadium in Glendale. I had an 8:15 PM appointment and the wife’s appointment was at 8:21 PM. We drove down early and had dinner. Dinner was over early so we decide to see if we can get the vaccine early. We arrived at the Stadium at 7:10 PM, an hour early. No problem, they waved us straight in.
The process started with cars arriving in a single line. The first volunteer checked our appointment documentation…

Any Open Slot Option Vaccine Appointment
Offer an "any open slot" option when registering for an appointment so that someone doesn't complete the process only to find that someone else took that open date while he or she was filling in the remaining screens.
Also, you still haven't put in a simple "NO APPOINTMENTS AVAILABLE" on the first page, so someone doesn't waste their time. Saying all January and February appointments are booked is not the same, especially when some open up and when March comes along.

My congrats to the teams that organized this event. It was so well organized and efficient! The volunteers and health care workers were all friendly and went out of their way to be helpful.

Please start vaccinations up north! Maricopa county is NOT the only location in AZ that needs to be vaccinated.
Set up vaccination sights in Yavapai county.

Horrible, awful, terrible!!! A complete disaster.
You royally screwed up! The old search engine worked... really worked. This search engine is ****. I don't want to go through Family Search or Ancestry to access records. Don't get me started on what I think of their mulish search engines. My suggestion Give us back the old search engine...

Timely MCDPH resident notification letter of positive Covid test & contact tracing info.
On 11/19/2020 I received a positive covid test result while residing in a Skilled Nursing facility following major surgery. The facility immediately moved me to a "covid unit" where I was quarantined until my release weeks later. I experienced no significant symptoms of infection during that time. On 2/15/2021 I received a letter at my home from MCDPH stating recent notification of my positive testing & the steps I should take including contact tracing. Recent? It's been 3 months. Perhaps MCDPH processes need to include a check of the date a person tests positive prior to sending out a useless…

Why isn't your testing data the same as reported by Johns Hopkins?
The daily testing trends reported by Johns Hopkins, for AZ, is materially different from yours. It seems that the numerator (positives) is often the same but the denominator (total tests) is grossly different. Why is that? We need full transparency and accuracy. See.... https://coronavirus.jhu.edu/testing/individual-states/arizona
