getting 404 page not found at https://ems.azdhs.gov/Application/CreateEligibility
I'm attempting to apply for my state EMT certification, I am already have my NREMT. I am trying to apply for jobs and the card from the state is all I am missing and cannot move forward till I have it. After entering my information and cliciking next on this page, https://ems.azdhs.gov/Application/CreateEligibility - it takes me to a page saying Error 404, file or directory not found and I cannot complete my application.

Is myIR not working?
Is https://az.myir.net/ not working or is it always down for maintenance at night??? Every day now for 3 to 4 weeks, nothing!

Smoking Violations Submittal - does not confirm success
Your Android and regular computer submission process does not confirm whether or not submission was successfully submitted - I tried numerous times and on multiple applications to submit my concern/complaint - could you please confirm that my concerns were received?

ATM at the Hana dispensary 2905 Northern Ave. Kingman AZ is stealing from patients.
This dispensary has so many problems with their ATM that they tell people to call ahead to verify it is operating. Tonite, 10/10/2017, the dispensary's ATM deducted $203 from my bank account and printed a receipt for no money dispensed. The position of the dispensary owners is that it is not their problem that the problem is with a company called Eclipse (800-390-0996) there is no one on duty but a voice mail indicates if you are the owner of the business, or an unhappy customer, call the bank and report it to them don't call the company that owns…

Convert the BRFSS survey to digital please!
I participated in the BRFSS by phone today and found the process to be almost unbearably painful. As a health care provider I appreciate the value of the data collected, but doing it as a scripted phone interview was overly long and difficult. I was able to find and read the questions on-line and should be able to complete the task more accurately and in a fraction of the time electronically. Please consider offering an electronic survey link in the future. I'm also curious why the most recent report available is from 2015? My interviewer had below average English speaking…

I cannot Enable your Payment
My computer does not load enable, play, do, whatever so I can pay for my address change onmy card. I do nor have SASFARUI I do not have chrome it ewill not losad. quick link for providers to log in
I am having difficulty finding the log in page for the DAVE site to be able to fill out a death certificate. once on the web page, it is not intuitive to be able to find the log in.

The Feedback & Support button on the right hand side of the screen covers up the last 2 characters of text that is actually on the page when in google chrome.
