    Dear sirs or Madam,
    I am writing to you on behalf of the Chinese Language Teachers Association in Arizona. We had a dear friend, teacher and role model, whose name is Sophia Lee, who was struggling with some unknown diseases and later lost contact.

  2. Viewing/Downloading of PDFs Not Working

    The option to download the historic birth and death certificates for genealogical purposes is not working properly. It attempts to generate the PDF but then the PDF pop-up just turns back into the search screen and the document never shows for viewing or downloading.

  3. Radiation individual portal access without initial e-mail

    (Ionizing radiation licensing) - Links to the individual licensing portal are provided to the professional via e-mail. Without access to the e-mail a user is unable to gain access to the portal via the website in the future (without needing to call for assistance). Please create / provide a method for a registered user to access the individual portal if they no longer have access to the initial e-mail.

  4. Practice of religion

    we wanted to share that there are experts who practice religion and they be in government and that it is not resolved that they have these issues where they are closed and to people really such as the mental health and sober fraud issue and the library and parks

  6. Who Is Backing These Things That We Are Not Supposed To Do?

    Marriage needs a priest, and I do think that. It is not even true that it is ok. We need to consider if the church backs this and if elements that are not good are too big of a risk and matters needs to be resolved.

  8. FIX THE GENEALOGY RECORDS PORTION OF YOUR SITE

    With the amount of people complaining about this broken part of the site for the last 3 years, you'd think someone would have at least attempted to figure out WHY those of us interested in these historic documents are stuck in a perpetual loop of searching and NOT BEING ABLE TO OPEN ANY of the search results!! I've tried several browsers now and NONE of them work to open these documents (Firefox wouldn't even let me verify I was a human and not a bot!!). I try to utilize this section of the site since the upgrade every few months…

  10. Allow Requests for Free Radon Test Kits

    You have a button on your website for people to request a free radon test kit but when I click it, it says that the form is unavailable for anyone that is outside of the organization. So much for free radon test kits.

  11. Get smarter people to run website

    We have been asking that you provide mobile users with a website that we can view. What are we paying you for. We can’t see the numbers at all now it was just numbers tested that was hidden before. Can you please please fix it so mobile users can get informed We don’t all walk around with our desktops.

    You can see all the bars on bar chart! Recent dates are not visible.

    Take a look at Maricopa county...they provide much more info in a very user friendly site.

  17. we would like more daily covid updates from the state leadership

    please give us more live updates on tv from the Governor regarding coronavirus as we are all affected economically as well as health wise and we need to hear from our leadership as we are hearing from other Govenors around the country who are keeping their people informed about what direction and actions their states are taking

  20. Please add hospital status to daily summary (like testing results were added)

    It was helpful when testing became "newsworthy" to add numerical testing summary to daily Summary page on the dashboard - thanks. I suggest now that hospital capacity is newsworthy, that this be added to summary, e.g. for 6/23

    COVID-19 2270 581
    Remaining Available 1120 200

    This is similar info to the helpful "overlay" graphs in the last two briefing slide sets. It summarizes data from "Hospital Bed Usage and Availability" and Hospital COVID-19 Specific Metrics" pages from same system, and sets aside the "Hospitalization" page information from a different system with significant time…
