  3. Cannot FIND WAYS to select/specify a designated CARE GIVER

    I live in Kearny, Arizona. Superior Organics Dispensary CLOSED. I would like to select a DESIGNATED CAREGIVER as a supplier. But I CANNOT FIND THE APPLICATION FORM that I need to fill out to identify and select my Designated Care Giver. Can you help?

    I am from the north East amd bought my son to Mohave Mental health for treatment of his heroin addiction. Not only did he have to wait two months to get suboxone prescribed. He finally got the prescription and was told not to take it until the following week we he went to his next doctors appointment, and to drive in there in withdrawls to take it in fron of the doctor. he sais what am i supposed to do over the next 5 days, continue to use heroin.   8. Why full price of $ 300 for renewal with incurable diseases

    Don t punish patients with expiring card and an ongoing incurable disease like Parkinson's with a $ 150 fee from a doctor who is not even the patients physician. He has also developed another cancer for which he is paying $ 2000 for the radiation. the products in dispensaries are expensive enough. Why can his physician/oncologist NOT write up the health condition and reason for a medical marijuana card yo avoid that fee? It is really unfair, just an additional income for doctors.   10. Fraudulent Billing Information

    When I submitted my insurance information for my newborn's screening test, I received a bill from ADHS saying that the patient could not be identified by the insurance as insured. I checked with my third-party administrator and found out that the claim had been accepted and was being processed. When I called your billing department to find out why that note had been added, the only response I got was that my third-party administrator "takes a long time" and that a note would be added to my account, after which I was hung up on. No mention was made of…

  18. quick link for providers to log in

    I am having difficulty finding the log in page for the DAVE site to be able to fill out a death certificate. once on the web page, it is not intuitive to be able to find the log in.

    I was looking for the online medical marijuana CME module. I went to azdhs.gov/cme, and it presents the option of "medical marijuana continuing medical education (CME) module." When I click on the link, it just sends me to a frequently asked questions page. Do you have a proper link to the module?
