  1. Produce departments at grocery stores should have strict requirements for employees hair.

If people stocking produce and handling our food have long hair they should be required to tie it back, wear a hairnet or a hat. There are any issues to consider with this requirement.

Grocery stores are icovered by the food code like restaurants and are nspected by county environmental health. While, the food code applies and people who work with food are supposed to “wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food” But even if they are, it’s good to remember that produce is grown fresh at the farm and there is the potential for contamination. We always recommend that you wash uncooked fruits and vegetables before you eat them

  3. control cost and difficulty in renewing MMC

Initially it cost $300 to get my MMC, $150 DR and $150 state registration. To renew my card it will cost the same because I need to go through DR verification again. I have severe, chronic back, neck pain and chronic headache. This has been a 12 yr problem which had me on Morphine, Methadone and Oxycontin. Thanks to MM I was able to quit all narcotic medication by using marijuana. I am a disabled vet and my VA DR can't write my referral for marijuana but can prescribe any narcotic which makes no sense to me. Now I need…

Thanks for sharing your story. At this point we don’t have the ability to change the fee schedule, but infomration like this will be used when we are able to make changes.

  4. I think they should hurry up with the sending out of cards my card has still not been sent to me.

I feel i was scammed out of a 150 by the azhds cuz i still havent recieved my card. what they should do is keep better documentation of the new cards being distributed for the new purchasers with the right paper work.

If you have a question about the status of an application, the best way to get help is to use the Live Support button that is on all the medical marijuana pages on azdhs.gov.

  8. Vital statistics question

Hello, I have a question/data request regarding data on abortions to Arizona residents by county of residence and age group for the period 1990 through 1999. Could you please provide contact information for Arizona Health Status and Vital Statistics division (preferably email). I tried to use suggested email for questions, comments or concerns for ADHS webmaster: webmaster@azdhs.gov but it does not work. Thanks.

  9. "25 Mile Rule" is Unconstitutional

What happens to patients cultivation rights when a dispensary opens within 25 miles of a patients address? How will the patient be noticed? How long do they have to stop cultivation?

    Please explain the plan!!

When the first dispensary OPENS near me I will file a LAWSUIT against that dispensary and The State of Arizona.

    Article 2 Section 13

Section 13. No law shall be enacted granting to any citizen, class of citizens, or corporation other than municipal, privileges or immunities which, upon the same terms, shall not equally belong to all citizens or…

Each qualified patient will be notified upon their annual renewal if a dispensary is open within 25 miles of where they live. If you renew with a request to grow and there is not a dispensary open within 25 miles you will be able to continue to grow. The final rules for the dispensaries are still being developed so you will want to check back to our agency website for new developments. At this time we do not have a date for the completion of the final rules and no dates for when the Department will be able to license dispensaries.
The 25-mile rule was part of the Initiative passed by the voters, as such is set in law.

  10. spreadsheet listing hearing trainings by county

    Hi!
I'm looking for a pure tone hearing training/recert. Instead of me calling all 8 or so Maricopa County trainers, wouldn't it be easier for the trainers to put info about trainings on the website, so we could see which works for us, then contact the trainer? Thanks.

T3, http://www.ag.arizona.edu/maricopa/t3/index.php is a train-the-trainer system that supports early childhood professionals, school personnel and others who work with children. T3 is a collaborative project that includes training in health, safety and hearing screening with the University of Arizona Cooperative Extension as the lead collaborator. Other members of the collaborative listed on their webpage include:
    • The University of Arizona Cooperative Extension
    • Arizona Department of Health Services, Bureau of Women and Children’s Health Services, Sensory Program, Bureau of Chronic Disease Prevention and Nutrition Services, Office of Child Care Licensure
    • Pima County Health Department
    • Maricopa County Department of Public Health
    • Arizona Department of Economic Security, Office of Child Care Administration
    • Central Arizona College (ECE)
    • Glendale Community College
If you to the website, simply click on the “Find a Trainer” and you will be provided a search engine by specialty as well as geographic area.…

  11. Some questions do not have answers that fit your screens

My Internal Medicine Board certification was in 1986, and HAS NO EXPIRATION. Therefore, I cannot volunteer on line to be part of the volunteer medical corp. You need a bypass for retired physicians with time, talent, and energy to serve.

Your timing was perfect. The comment came in right when we were working on revamping the form. It should be taken care of now.

  12. Post Important info on website..

At this time there is NO way for a patient to know when a dispensary might open within 25 miles of their residence. There should be a place on the Dept. of Health website listing dispensaries and a circle of the area (25 miles) showing what patients will not be able to cultivate.
The current measure notices the patient upon their annual renewal if a dispensary is open within 25 miles of where they live. What about all the money invested in cultivating your own medicine? The current measures will create a BURDEN on the patient.

At this time, ADHS has no idea where dispensaries will open either. When a dispensary begins operating, people living within the 25-mile radius will be notified.

If the Customer Feedback button was covering the left navigation of azdhs.gov, we assume that you have your screen resolution set in the neighborhood of 600 × 800 pixels. Only 3% of our users are in that range. However, we did receive a few complaints, so we’ve moved the button to the right side, away from the navigation. If you continue to experience an issue, you may want to increase your screen resolution to a more common setting such as 1024 × 768 or greater. We see that you did use the button, gave us feedback and we thank you!

  16. There needs to be verified training for all staff on both physical and mental illness goes together. A person is a whole person.

Training could be given by consumers as well as developed by them. Training could encompass hearing, speech, developmental disabilities, mobility issues as well as learning. Perhaps having staff experience different disabilities. ADA seems to be forgotten.

Thank you for sharing your idea. Arizona statutes prescribe who licenses what types of people. At ADHS, we license a few professions and several types of facilities It seems like your idea would be good to provide to the licensing boards. We’ll share your idea with our licensing staff.

  17. I am dumb founded

My Idea is this, I can not believe what the ADHS has put me through to get my deceased husband's death certificate. I have been trying to get this certificate since July of 2011. it is funny how I had to do all the leg work for this department, just to get a lovely letter saying that i was not listed as the spouse on the death certificate and was not eligible as spouse. I had to laugh so hard at the fact that the state of Arizona Department Vital Records did not recognize the legal Marriage Certificate issued to…

Thank you for providing additional information to our staff. We are addressing your situation.

The Arizona Office of Vital Records issues only birth and death certificates. It does not issue marriage certificates.
To be eligible as the spouse to receive a certified copy of a death certificate, the surviving spouse must provide documentation that they were in fact married to the decedent at the time of death.
A marriage certificate alone does not provide sufficient proof that the marriage was still intact at the time of death. In this particular case the death certificate marital status must be in conflict with the applicant’s claim of being the spouse.
If the information regarding the marital status (which is provided by the informant on the death record) is in error, the applicant may file for a correction to the death record with the Office of Vital Record’s Death Registry.
    If the marital…

ADHS did not set the 25 mile rule. It was in the initiative passed by the voters and made law. You can find it 36-2804.02. Registration of qualifying patients and designated caregivers. We will enforce that part of the law once dispensaries are in operation, unless there is a change in the statute.

  20. Needing a birth certificate

I live in Sioux City, IA, and I need to get a state ID, and a drivers license. In the state of Iowa you need your birth certificate and SSC to obtain this. I was born in Phoenix, AZ and do not have my original birth certificate. I came to this website and called the office to try and get it mailed to me, but you cant unless you have the necessary identification. which I do not. For applicants who do not have the required identification required for an Arizona birth certificate they can complete the Application for an Arizona Birth Certificate and have their signature notarized. The notarized application and the fee can then be submitted to the State Office of Vital Records for processing. The birth certificate application form can be found at
    http://www.azdhs.gov/vitalrcd/index.htm
