Why full price of $ 300 for renewal with incurable diseases
Don t punish patients with expiring card and an ongoing incurable disease like Parkinson's with a $ 150 fee from a doctor who is not even the patients physician. He has also developed another cancer for which he is paying $ 2000 for the radiation. the products in dispensaries are expensive enough. Why can his physician/oncologist NOT write up the health condition and reason for a medical marijuana card yo avoid that fee? It is really unfair, just an additional income for doctors. Fraudulent Billing Information
When I submitted my insurance information for my newborn's screening test, I received a bill from ADHS saying that the patient could not be identified by the insurance as insured. I checked with my third-party administrator and found out that the claim had been accepted and was being processed. When I called your billing department to find out why that note had been added, the only response I got was that my third-party administrator "takes a long time" and that a note would be added to my account, after which I was hung up on. Suggestion for CITING/REFERENCES
I use your website pages to do school research papers but I never am able to find who wrote the topic on certain things or what year it was written, is there any way this can be updated into the webpages on the website?

quick link for providers to log in
I am having difficulty finding the log in page for the DAVE site to be able to fill out a death certificate. once on the web page, it is not intuitive to be able to find the log in.

Medical Marijuana CME
I was looking for the online medical marijuana CME module. I went to azdhs.gov/cme, and it presents the option of "medical marijuana continuing medical education (CME) module." When I click on the link, it just sends me to a frequently asked questions page. Do you have a proper link to the module?

That hospital in Ajo was closed down many years ago so if there is no central recording office for the State, where are those hospital records archived so that a copy of the original birth certificate can be accessed?1 vote -

our contact section refuses to send my message stating that cookies are disabled. That is not so. Cookies are enabled so your tech needs to get to work and fix whatever problem your site has. Why does your Department not have direct email access as the other departments do?

Trauma Surgical Facilities
In recent years, Account Pamela & Associates conducted a feasibility study on Trauma Surgical Facilities in Arizona. More recent research reveal the southern areas of Phoenix are without adequate medical facilities and hospitals. Specifically, the Ahwatukee and South Mountain area is currently observed for Trauma Surgical Facilities. FYI: The nearest hospital in some locations are between 10 and 17 miles away. Account Pamela & Associates in coming months will propose Trauma Surgical Facilities and other Life Care Facilities for the growing areas of Arizona.

Dysfunctional Application and Broken Email Link
Trying to use Cancer Cluster map by Neighborhood, and the application does not open. The link to leave feedback on the application is broken. This is important to me; please fix. I already updated the Adobe Flash. That is not the issue.
