Dear sirs or Madam,
I am writing to you on behalf of the Chinese Language Teachers Association in Arizona. We had a dear friend, teacher and role model, whose name is Sophia Lee, who was struggling with some unknown diseases and later lost contact.
Some people said she passed away and some people said she moved out of the country. We would like to seek some assistance to see whether you hold any death record of this person. I think she is in her late 50s or early 60s, she doesn't have any family members here in the USA. Last time we knew about her work place would be Scottsdale Unified School District.
