    remove or clarify students
    if we contract with a school to be their clinical site, the school is responsible for the students file/ information as stated in the rule
    A better expectation for a facility who contracts and does not manage their own student population whould be, we are expected to keep a list of the student's name, address, phone number and dates they are in the building for clinical training

  2. all I want to do is replace a lost medical marijuana card, why so difficult

    Why can't I come to dhs and do this, not everyone is a computer geek....or even has a computer...

    We’ve streamlined our card application and renewal process using the internet and have had great success. Compared to other states that use a paper process, we are able to provide the cards within days rather than weeks or months. If you are having specific problems, you can use the live chat system during normal state work hours and we will help you with the process.

  3. MMJ for PTSD

    As a 63-year-old Vietnam Vet I can personally attest to the profound affect marijuana has had on this condition. Ive had PTSD for 43 years but the government refused to recognize it even existed until recently. Marijuana keeps me from going over the edge and allows me to function as a productive member of society. 43 years! PTSD is real MMJ helps relieve symptoms. Come on, get it right for a change.

  5. Public Health Services - Transparency for charges

    It is not common knowledge (or even practical) to look on your website for ambulance charges in an emergency. I had to use an ambulance in July of this year and I learned AFTER I used this service that it cost me $1,000. Health care policy info along with local ambulance charges should be loaded into emergency responder computers for real time communication of charges. In a free market, customers are made aware of charges for purchases and it should not be different with purchasing health services! Transparency of charges should be made up front!

    That’s an interesting solution. It would require a change in law to require ambulance companies to do that. You might want to talk to your state legislators about the idea.

    I have been trying to contact you about a diseased cat in Quartzsite that gave a man a nearly deadly disease. I cannot send an e-mail with info.

  8. Would it be possible to have a link specificaly for forms?

    Found exactly what I needed, however, it would have been much easier to click a link for 'forms', and look it up there. :) Thank you for making following guidelines easier on us all! Very appriciated.

    Thank you for your question about court ordered paternity.

    The Arizona Department of Economic Services (DES) publishes valuable information about establishing paternity. This information can be located on the following website; https://www.azdes.gov/main.aspx?menu=24&id=3777

    As mentioned on the DES website, parents may choose to go through the Arizona court system to resolve any issues. This choice may involve attorney fees, court costs and filing fees. Detailed information can be found through your County Superior Court office.   10. Application incomplete?

    Please, I have an inoperable brain tumor and filled out all the necessary documents over 2 weeks ago. My credit card was even charged the fee.
    I checked on status and apparently there was something mailed to me that was necessary. I did not receive this!

  11. advocate for the patient

    why doesn't the ADHS not have an avenue for the patient to complain if they feel they have suffered as a result of being given MRSA at the hospital and it caused many levels of sufferings amputations and more.

  12. Impossible to correct or add to application

    It has been 2 weeks, my credit card charged and an email explaining an error was received yesterday. However, when I get on ADHS, there is no record of me. It would be nice to correct the issue that way as the hyperlink in the email does nothing.
    When you are gravely ill, this is quite frustrating.

  15. Improve Vital Records- Adoption Process

    It is a disgrace that adoptive parents need to wait this long to get an updated birth certificate. My case it has been 1 month and 2 weeks. Not only the State make it horrible and difficult to finalize the Adoption process, but on top we need to deal with incompetent staff that blames supervisor for the delay in approving amendments. Why is this difficult should be a happy process. Perhaps your staff have no IDEA how many things get delayed because this important document is not available. Also, it is almost impossible to reach a supervisor in this department.

    Various factors can potentially impact the timely issuance of an amended birth certificate after adoption.

  17. NM Patients obtain MMJ card from AZ-Reciprocity

    I live 10 mi. east of central az(I'm in ZUNI,NM, east of "Wicth Wells" on hwy.53)- I am 35 mi from St. John's, AZ. I think I should be able to access AZ MMJ Dr's. and Dispencaries. It should go BOTH WAYS, too. I believe I may presently be allowed to visit az dispencaries w/nm lic (and get az lic for mmj from az drs). Please, respond asap-I'm trying to get nm mmj lic. by this Christmas (yeah, good luck to me, unlikely at best)-I have 14 conditions, but would use CHRONIC PAIN, Painful Per. Nuropathy, R.     Cardholders from other states cannot purchase medical marijuana from an Arizona dispensary. In order to purchase from an Arizona dispensary, you will need to apply for an Arizona Medical Marijuana Qualifying Patient card. Please read through the patient application instructions and checklist on our website: http://www.azdhs.gov/medicalmarijuana/patients/adult.htm This is also where you can access the link to apply for a patient card.

  19. Report form for drug-seeking behaviors

    I have an acquaintance who travels to various hospitals in the Surprise area every month or so claiming to have a migraine in order to obtain narcotic medication. She has been to at least 6 different hospitals in the last 8 months, and she is successful in obtaining a prescription for narcotics nearly every time. When one hospital refuses her, she simply travels to the next, and the next, until she gets her fix. She was found out in both FL and NM for similar behavior and she was no longer able to waste the emergency room staff's time or…
