    You sent me an email requesting that I take a survey regarding the new MMJ digital license. I could not complete the survey because as soon as I answered one question it thanked me for completing the survey (which I could not complete). I suggest you check on the survey and see if it is working properly.

  4. Update your page on medical marijuana

    We waited 6 days for the 'welcome' email saying that we could access my grandmother's med card. I call and get told "we don't send that email anymore, just log into the portal". How absolutely disgusting that no one told us this, meanwhile my grandmother has stage 4 cancer and is waiting for her e-card to arrive in her email. Change your website. People are dying.

    Unfortunately, the Smart & Final grocery store on Broadway and McClintock in Tempe, Arizona has now closed. Please expand the Food Desert green to include this area.

  8. Our hospital does not have a new contract yet available with any company for supplying us with new dosimeters starting Jan.1, 2020.

    Our hospital does not have a new contract available for our dosimeters
    as of Jan. 1 , 2020. What does that mean concerning usage of our xray
    equipment and personnel if we do not have current dosimeters to use?

  12. Make a way for clients in transitition to post updates to their status. Which I have been trying to do now for an hour and a half.

    It is self-explanatory. At least that's what I have been told when explaing what I'm doing here. I'm getting validate current active card to update your information. Or,

    I Jose Sanchez went to Arizona Organix located near 51st ave and Glendale dispensary yesterday and they gave my MMC to another patient, I still not yet recieved any information regarding my card!! how do i go about getting it back, or what do I do!!

    I have NO idea how to upload photos and documents required to renew my license. PLEASE BE SPECIFIC ON HOW TO DO THIS. I'm not tech-savvy. I suggest you include information on how to do upload on your website.

  17. Do not limit some information by lack of county selection

    I tried to subscribe to Arizona Public Health newsletters, partly out of concern for things like the coronavirus long~term (I had no idea it was extant even though it seemed like there would be more with things like Ebola and Zika) and then it seemed that there was a confirmation eMail sent to me and then no newsletters?

    I think it was the lack of selection of applicable county lived in as I was not able to answer that question legally, as I live in Michigan though one cousin did and one does, and I am interested in some things…

  18. Make it easier. Coming here in my condition isn't my pleasure.

    In Dec. 2019 I received email to update things on your sight. When I did, I also downloaded last year's card here which is now expired. I had renewed prior to exp. and my new card is in your system. I'm not able to remove my expired card or locate my renewal. I need assurance I can still purchase my medication. I need sleep, badly.
