im being abused by my provider nobody will take my complaint

i think that you should make a way to order the birth certicates online to get them send by mail
there should be more ways of getting the birth certificates aside of mailing the form and getting it in the mail or going in person there should be a way to order them online and receive it by mail

You need to provide printable versions of the info on this site.
I am trying to provide the information on the Apply for a Birth Certificate page to a client, but I can't print off the important information found on the side bar, which includes your physical and mailing address. This side bar does not print.
1 voteThank you for your comment. Our system is not set up for printing the information on the left navigation. The best option would be to take a screen grab of the page and print that. You could also highlight the information and paste into an email or document for printing.

This will help Incident Command, new Public Health and new HCW or agencies download a current listing as needed.

Thank you for writing. On the left hand side of the website, there is a link to fee schedules. Prices change depending on where you ask for a copy of the certificate so we cannot just say it costs $xx.

I don't have an idea or suggestion. I'm just trying to find someone to answer my questions about Home Health Agency Regulations.
Specifically, dealing with Supportive Services R9-10-1211 B 1 b.
An administrator may allow: A personnel member who is not a home health aide to perform personal care services. Please clarify then, who this careprovider is?
and
B 2b. A registered nurse i. - v. Can a LPN make the actual visit and collect data which the RN reviews to assess the pt's need for supportive services? Waiver for LPN
We (The New Foundation) are a level 1 Residential/inpatient facility (not hospital-based).
We would like to know if it is possible to get a waiver an an LPN (who is currently enrolled in school to get her RN degree) to work the overnight shift with an RN on-call. She has been employed with us for over two years.

The RBHA map on the DBHS website shows MMIC on the map itself. However, the key below the map still reads "Magellan" in the coloring that should reflect GSA 6- MMIC.

I misplaced the emails sent to me. Please resend them. I have a current Dr.s Rx.
I had the emails that were sent to me. Having my daughter uploading the information, they were lost. Please email me how I give the information needed to finish the renewal process. I'd hate to loose the money it took me so long to send. That is why I'm having this trouble. It took me 6 months to get the monies together. Sending an outdated Rx.

make forms for CSA title XIX application and renewal available
I click on the links in 406; all I get is redirected back to 406.... I need the actual forms! we used to click on the attachment number and the form would come up. can not find my forms.... attachment 1, attachment 3, attachment 4, attachment 6..... my idea is to connect the links to the pdf forms? thank you!

Correct the flaws in Our laws
Local jurisdictions fall under the stated law the same way as anyone else are they not ? I recently returned back to the state from California and am a qualified medical person. It seems to me the bureaucracy involved in this program is excessively burdening to the people of this state.I attended the meetings in Flagstaff and as well as Phoenix. And specifically expressed the 10 foot wall. As it is not possibly realistic for a person on disability to afford the overwhelming cost of such a monumental requirement.I was under the impression that was corrected in the statute as…

Please be advised a caller from 336-639-9871 is claiming to be calling from the AZ Dept of Health regarding "Medical Compensation." Caller gives name as Peter WIlliams and asks a series of personal questions then hangs up when credentials are demanded.
