Allow Requests for Free Radon Test Kits

You have a button on your website for people to request a free radon test kit but when I click it, it says that the form is unavailable for anyone that is outside of the organization. So much for free radon test kits.

    The google form for requesting test kits is misconfigured to require permission. Please fix the configuration to be public or allow a way to request access.
