ang a caregiver to someone who has a card already
when my husband initialy had gotten his medical marijuanna interview with the Dr. we didnt realalize that I had to be designated as a caregiver at that time and now how am I supposed to be added now you people need to have a seperate form for caregivers to be added to those who already have their cards please.

on the AMMA page the list on the left hand side that has qualifing patients
caregivers, etc. one tab says forms. There is a form there for add/replace caregiver. I imagine that you would submit that. Although the form doesn't look like much, I would write in all necessary information on it and send it. -

When I got my card I was asked if I wanted to cultivate, I understand that if you do not request to you can not grow any plants but must assign a caretaker to do it for you. Ifyou want to be a caretaker fill out the form and pay them $200.00 for the opportunity.

Given, that was a run-on mouthful. But the question is an interesting one: If the patient originally noted on their card that he or she will cultivate, how does that patient, after the card arrives, change the card (for $10 I assume) from cultivate to appoint a caregiver?
