change request forms
With the dispensary applications being suspended, people who were expecting to buy from them will now be looking for a caregiver or look to cultivate. The change request forms will help a lot of people who chose not to cultivate, obtain medication. I have seen similar concern from many patients. So I'm suggesting that they be put out as quick as possible. Other than that I would say that you guys are doing an amazing job.
The change forms are available on the the application page on the website at this time. Just go into the the patient application, agree to the terms and you will see the new option.

We anticipate that the Medical Marijuana Program online change feature to be available in the next few weeks. "PLEASE make the change forms available". I've made numerous calls left voice mails, not once have I received a call back or had a human voice answer. Maybe this is the best your department can do, however it is GROSSLY UNACCEPTABLE! If my employees didn't answer the phone, make the call backs the answering machine proports or make a simple form available why would I employ them? Whom employs you? Not trying to make enemies. Just becoming less tolerant of lack of excellence.

I have been looking for change forms since I received my permanent card almost a month ago. I also asked a Live Support operator if there is an estimated time when the forms will be available. Although very courteous and prompt, the operator had no updates regarding the forms. I hope patients' feedback will help bring the importance of the forms to the attention of ADHS because with no dispensaries available this matter is becoming urgent.

I also agree the Caregivers should definitly be Notified and even involved in the decision, but ultimatly it should be the patients choice. I would advice that the caregiver have a waiting list of patients and just add a new patient in the place of the removed patient on the caregivers current patient list.

The forms need to be available soon i need my meds as a patient this is more important to me then a caregivers money. I am confident my Caregiver can produce but if changes in the way distrbution is being handled effect their ability to supply i may need to change to self grower.

when the change request forms are up, patient might be able to change caregiver. That may cause some damage for the caregiver if the patient simply chose not to let their caregiver know and caregiver have no way to check. Also the caregiver might be in the middle of growing, changing caregiver at this time might do financial damage for caregiver since they have already invest in the grow room and in the middle of growing.
Caregiver paid a higher fee to be certified than patient and if the patient chose to change caregiver, the caregiver just lost their certified fee, fingerprints fee.It would be very dangerous for the caregiver who was approved to cultivate to think that they can cultivate for their patient, have the correct amount of plants and end up with "breaking the law," get charge with having too many plants because one of their patient change caregiver without telling them. So a caregiver who have no criminal record is now becoming a criminal.
I think on the caregiver change request form, it should ask for the reason why patient opt to change caregiver. For example: misconduct by caregiver or if caregiver move, etc...and give caregiver 5-10 days to response. Then the state decide if approved to change or not.
or require both caregiver and patient signature to change.
