change request forms
With the dispensary applications being suspended, people who were expecting to buy from them will now be looking for a caregiver or look to cultivate. The change request forms will help a lot of people who chose not to cultivate, obtain medication. I have seen similar concern from many patients. So I'm suggesting that they be put out as quick as possible. Other than that I would say that you guys are doing an amazing job.
The change forms are available on the the application page on the website at this time. Just go into the the patient application, agree to the terms and you will see the new option.

We anticipate that the Medical Marijuana Program online change feature to be available in the next few weeks. I have recently moved and would appreciate an online change of address form and an automated phone system to change address. I've been online a half hour and can't find a way to do this. I don't see chat either. Please assist.

"PLEASE make the change forms available". I've made numerous calls left voice mails, not once have I received a call back or had a human voice answer. Maybe this is the best your department can do, however it is GROSSLY UNACCEPTABLE! If my employees didn't answer the phone, make the call backs the answering machine proports or make a simple form available why would I employ them? Whom employs you? Not trying to make enemies. Just becoming less tolerant of lack of excellence.

I got a follow up email after 2 emails to m2programsupport@azdhs.gov and a phone call, thanks! I was told things should be fixed if not call this # 602-364-1$%4, It must be on mute because it well ring for 3 mins. at a ***** then hang up. I've rang that # for about an hour now and no answer!

this is just in response to the comment by "caregiver" - your idea would open a huge can of worms, it's kinda ridiculous to expect the state to regulate all the minor things like that, if your patient decides they want to change caregivers and that causes problems for you, that is your problem. you may want to hire a lawyer to draft up a contract that you and your patient can both sign, which will prevent the patient from switching their caregiver in the middle of your grow, if that is a major concern for you.

How ironic patients have spent all this money to become legal registered patients and now with the hold on dispensaries we are forced to break the law to bring seeds, clones, and medical grade medicine across state lines making us do criminal acts or continuiong using the mold street ****. Also the law clearly tells patients where not to use medicine and not to transfer to anyone without a card but gives nothing to where we can medicate and how we are to obtain medication now that you stopped dispensaries! There is a reason people buy their fruits and vegatbles in a store, it is hard as heck to grow your own. With the enclosure restriction patients are required to spend thousands on grow lights, humidifyiers, meters, nutrients, fans, ventilation and hello-we are sick people on low incomes.

So why are the forms not available online right now? How are people supposed to add a caregiver if you don't have this online? Also please clarify the fees for the caregiver. Laura Oxley has stated in an email to me that caregivers are expected to pay up to $1,000 if they sign up their maximum allotment of patients. So $200 per patient which is not stated in your Rules. The only rule I see is a Caregiver is to pay $200 fee for their annual card. Where is it written in your rules that a Caregiver is to pay an additional $200 per annual card fee? Nowhere and this is your error once again and you need to refund all the patients the excess fees you've been charging them since this is bordering on fraud.
