I need a replacement Card
Ive lost my Medical Marijuana card and ive gone on here before and found the form to replace it, but didnt have the money at the time. Now that ive come back to order it, the form can no longer be found. Please help. Patient ID: 0118084QPDV345267001

Its real easy. Costs $10. Might need to call state or your mmj cert clinic to get your ID. Also if you have an old card available, your number stays the same but the last number changes from 1 to 2.

Hi my husband lost his card so I oder him a new one and played the 10 fee. I have 2 questions, 1.) How long does it take to come in the mail? 2.) If there any dispensaries that would allow him to get his medications with just a copy of his lost card? Or maybe some delivery place when he uploads a pic of his card?

You all take multiple days to respond to emails. It is ridiculous when trying to maintain a chronically-ill population.

When a cardholder requests a new card, it is the procedure of the department to regenerate a new card. When this happens, the old card status is automatically deactivated and a new card along with a new card number is activated and mailed to the cardholder. Because the new card is immediately activated upon issuance, the old card must be deactivated to prevent a cardholder from having two active cards at one time.

I’ve lost my card. Need a new card # to get another but after 3 days no one has responded to my request. It should not be this card to get a replacement.
