After watching the news about the needy foster families that depend on those checks from the State that now are delayed.
With all our charities, can any of them step in to help these families on a one time basis for their rent rent/or food for the 3 week delay of checks from the state. Maybe there would be enough generous people to help these famiies taking in foster children where the tv stations could go and ask the public for help to a special bank account. It's such a shame that these families can't have piece of mind on the Holiday
1 voteThis request/ suggestion would be best handled by the Arizona Agency that has the Foster Care Program. That is Arizona Department of Economic Security.

Be more specific on application for stolen card
I filled out the form , paid with a credit card , but Im not sure if the process is complete or if I need to go somewhere to finish this now . All it says is print out my copy?
1 voteThank you for contacting the Arizona Department of Health Services, Medical Marijuana Program. We apologize for the confusion you experienced with your replacement request. Normally after you pay for the lost or stolen application the system will give you the option to print out your receipt. After that page, the system will normally give you a message that your application has been successfully submitted. We would be happy to verify the status of your application. Please call the MM Program at (602) 364-1793 or email at M2programsupport@azdhs.gov . In order for us to verify your identity and provide a status update you will need to provide your date of birth, address, telephone number, and driver’s license or state ID number (whichever you used on the application).
Again we apologize for the inconvenience you experienced and we look forward to further assisting you.

Bring back the website that allowed us to Search different types of facilities: ALH, Medical, BH, etc.
Being able to search for Address, Phone, Fax, etc., for a medical facility, SNF, or assisted living home or facility where our members are living is extremely valuable information. I use it for my job all the time. Please bring back the page that was at http://www.azdhs.gov/search/. Nothing on that new page takes us to ADHS Search for facilities.

Link to WIC Prescription
It would be extremely helpful to have an obvious link to printable WIC prescriptions so that physicians can easily find them instead of searching through the whole site.
1 voteThank you for the suggestion. The new WIC Website has a special section for just for Physicians with information on the documentation needed for WIC Prescriptions including the printable form. You may want to bookmark that particular webpage or just look for the “Physician” box on the home page to get there quickly. I can'te get the address field to work
The map on your website is very helpful but when I put in an address to try and see what CHAA it is in it doesn't do anything. I am using google chrome

Can I take it in my luggage?
Hello and good morning,...I am flying to NC...driving to PA then flying out of PA to come home!...my question is can I take of marijuana on a plane in my luggage on my trip?
I was going to put my flower and my supplies and a copy of my card in my luggage when I check it...I do not wish to make the evening news...just have my medicine when I get there.1 vote -

keep it on line,as it's to hard for us older people to go down town.
It is so much easier to view or order on line.. We need this for all vital records.

We cannot post one fee schedule on the front page because fees may be different depending on where you apply for the certificate. There is a page that lists the fees for birth certificates linked off the same page as the application.

What the Heck????
My Mom passed in 1988. What a mess to get a death certificate. I do not know the date and month, that was 25 years ago. I can provide all of the information if anyone would pick up a phone and listen. And do not tell me that you get thousands of requests every day. This is a personal matter, and being treated like some terrorist appals me. It is about my MOTHER. Thank You for your lack of concern, and lack of service to your public. refused service at YHS Cottonwood because of posts made to public social media page,FACEBOOK
I was refused access to my medical marijuana because of posts I made to public social media. The medical director, owner, and the manager are punishing me for posted on their public social media page. If comments were nor permitted, they should have locked comments, rather than penalize me for expressing my freedom of speech on their public social media page. Attempts tp resolve this have fallen on deaf ears, leaving me unable to get my medicine. PLEASE HELP!

Genealogy Death Records
I've attempted several times to search for my relatives death record. Carl G Holz died 19 May 1908 in Maricopa County Arizona. Each time I begin the search, the website closes my web browser.
