What the Heck????
My Mom passed in 1988. What a mess to get a death certificate. I do not know the date and month, that was 25 years ago. I can provide all of the information if anyone would pick up a phone and listen. And do not tell me that you get thousands of requests every day. refused service at YHS Cottonwood because of posts made to public social media page,FACEBOOK
I was refused access to my medical marijuana because of posts I made to public social media. The medical director, owner, and the manager are punishing me for posted on their public social media page. If comments were nor permitted, they should have locked comments, rather than penalize me for expressing my freedom of speech on their public social media page. Attempts tp resolve this have fallen on deaf ears, leaving me unable to get my medicine. PLEASE HELP!

Seeings how the dispensary in Ajo hasn't opened or hung out a sign or sold a single gram in 20 days how about giving us back our grow rights !

I've attempted several times to search for my relatives death record. Carl G Holz died 19 May 1908 in Maricopa County Arizona. Each time I begin the search, the website closes my web browser.

Women's Right to Know Page
On the resources on the Women's Right to Know page, you include crisis pregnancy centers, but no clinics that provide abortions. Crisis pregnancy centers are typically religiously affiliated, politically driven, and often provide inaccurate and/or biased information, such as linking abortion to cancer (which evidence does not support). If you're going to include these, can you minimally also provide links to the local Planned Parenthoods, as these provide both counseling and abortion services? Women have the right to access to this information in one location. Thanks!

FAQ Answers will not stay open so I can read the solution. Is this an inside joke so to waste users time? Click on the answer and it opens then closes, this is by design it appears. Maybe it is a bad idea to play games, alot of good IT people out ther.

index of marriage records by name
I am looking for my grandparents marriage license. They were possibly married in Phoenix in 1929. Isn't there a data base somewhere of names and marriage info. to at least learn if they were married here?
1 voteThe State Department of Health Services is not the keeper of the marriage license records. We do keep a total number, but you would need to check with the County in which they were married for more information.

Need a site for licensed persons and documentation and healt records "needs' to be compliant under the new rulings site..
how about a keep it simple for updates for licens healtcare personell to keep abreast of the legislation rulings for those of us that are not savey and just don't want to be accused of breaking the laws..

open the vital records office to the public!!!!!!
This office is essential! There are instances when time is an issue and without being able to get immediate assistance things take an immediate turn for the worse and live are greatly impacted for the worse. This comes from personal experience. Yet again our government failing us and our taxes are for nought.

Day 24 and still not open ! Can we mail in a petition? A lot of card holders can't afford a computer . It cost me $300 the day it said it was opened I only make $550 a month on ssi . I thought this program is for the poor

Information on site is unhelpful
why not provide an easy way to access records (in particular marriage and divorce) - looking for two divorces, one in about 1919 and another in 1946 or 47. Have no idea how to start, other than paying a service to do this. Your on-line records for older birth and death certificates works well. Why not do the same with other records?
1 voteMarriage and divorce records are not available from the Arizona Department of Health Services, Office of Vital Records. Marriage and Divorce records in Arizona are maintained by the Clerk of the Superior Court in the county where the event occurred.

Look into the populus who were raised in Morenci AZ in the 60's, 70's and 80's .
There have been a multitude of my former classmates who either died young and still lived in Morenci or who have moved away and have passed from some form of cancer or are currently suffering, or are survivors of this disease. There has to be a correlation to this. There are too many occurences. Either caused by the copper mines air quality at that time or the occurrence of radiation fall out in that area.

Require Vitalchek to follow published instructions
My son was not issued a birth cert after following instructions provided by Vitalchek. I was charged for a birth cert that they will not process without a state issued Id. The notarized form was received by them. I must now file as a parent because he needs the bc for a state issued id.

stop the growig rights
Ajo dispensary is still not open ! How about giving the people back their growing rights ! If they can't serve the people why are they can't we grow?
