Where is my renewal card? It has been approved and over 2 weeks now and no card.
Where is my renewal card? It has been approved and over 2 weeks now and no card. A friend of mine went the same day for the first time and she is is still without a card to get any medicine. Why can't the dispensary's be able to see that she has been approved just no card yet so that she could at least get some relief. THIS IS MEDICINE! Just because it is not the chemicals that all governments get a kick back from the pharmacies does not mean it does not work. The states should not complain about…

in regards to the massive bed bug infestation at Church on the Street
The bed bug infestation at Church on the Street at 3210 NW. grand avenue must be in tens of millions of bugs which must be infecting the entire city and state of arizona with some 100 individuals going through their program every few months-most leaving because of the hundreds of bed bug bites they receive every night in the 100 or so rooms. The entire facility needs to be stripped and decontaminated. Having lived there 6 months, I and my fellow victims received thousands of bites-one room that was bombed with cedar oil killed 2 thousands bugs that we counted…

Clarify how you can "provide an uninterrupted supply of meds".
How can I maintain an "uninterrupted supply of medication," when I can only possess 2.5 onces at a time. It takes 6-8 months for a plant to mature and you would have to have a plant mature every 2 weeks to maintain this. What am supposed to do with the excess, as a plant will produce far more then that at one time. Can I drop off the extra with you or at the police station and pick up my 2.5 ounces bi-weekly? I am trying to abide by the law, but unable to figure out how to do this…

ADHS has many programs and can only feature a few at a time. The Tobacco Cessation program is important to us and hasn’t been highlighted lately. We will consider moving it up for awhile. Can you move it to the right side of the screen? or hang it from the top, or pop it up from the bottom? Or make it possible to move it out of the way... or provide an option to close it if we're not interested...
2 votesIf the Customer Feedback button was covering the left navigation of azdhs.gov, we assume that you have your screen resolution set in the neighborhood of 600 × 800 pixels. Only 3% of our users are in that range. However, we did receive a few complaints, so we’ve moved the button to the right side, away from the navigation. If you continue to experience an issue, you may want to increase your screen resolution to a more common setting such as 1024 × 768 or greater. We see that you did use the button, gave us feedback and we thank you!

Question: Unable to attain a letter from the local governing body
How will AZDHS handle CHAAs where the city or country refuse to sign a letter stating that the location qualifies. This refusal which is currently the case in several locations is driven by the fear that Jan Brewer's lawsuit instilled in various governing bodies.

Make it easier to find information and applications.
The site is quite convoluted & hard to search.
You could have the mailing address and phone number on the citizen status form & have that link on the online renwal application not separate.

You need a cover sheet that says what you need and have forms available on line forexample you are asking for director information where is your form
New form doesntask for yourarticlesand board of director inf etc this was clear in your old forms my came back with a ted letter but it wasnt asked for in your forms

Need for license verification
As an employer we need to verify the status of an EMT-P license. I cannot see any where on your website that it can be done.
