  1. Health club question

    Can someone please explain why it is ok to have four people sit around a nine square foot table in a restaurant, with no masks on, yet it is not ok for two people to play racquetball on an 800 square foot, fully enclosed court with full face plastic shields (not a cloth face covering)? I would like to hear directly from someone from the health department because getting someone on the phone seems nearly impossible. Thank you.

  2. I requested an application for a copy of my birth certificate via the phone & have not received it yet.

    please make it easier to print an application for a copy of a birth certificate. The Microsoft Reader wouldn't open for me.

  3. Information on Adverse Health Events?

    Does ADHS collect information on "Never Events" (Otherwise known as "Adverse Health Events")? If so, will this information ever available? Since this type of data tracks very serious errors/issues, it would be great to have access information regarding hospital incidents.

  5. filed a complaint in 02/17 regarding Phoenix Baptist Hospital negative treatment, and civil rights violation, of patient privacy.

    been several months and have not heard a response from the public health regarding this matter i am curious why know one has contacted me with the status of this investigation.   6. Hire a completely new staff

    I can get better customer service from a lobster in the middle of the Atlantic ocean in the pit of a whale shark's stomach that has croaked over and been eaten by 19 great white sharks. Your customer services are worse than a banana peel that has been sitting in the garbage can in my bathroom for the last 7 months. FIX IT

  7. Convert the BRFSS survey to digital please!

    I participated in the BRFSS by phone today and found the process to be almost unbearably painful. As a health care provider I appreciate the value of the data collected, but doing it as a scripted phone interview was overly long and difficult. I was able to find and read the questions on-line and should be able to complete the task more accurately and in a fraction of the time electronically. Please consider offering an electronic survey link in the future. I'm also curious why the most recent report available is from 2015? My interviewer had below average English speaking…

  10. make a publicly available version of the data set

    I am currently working with ASU for my capstone and it would be very beneficial to me if there was access to the original format of the dashboard. Also, employing the community to help make dashboards or even making a sample of the data for people to work with could be a great way to engage the public.

    The Clovid-19 data interface is beautiful. Basically breaks the touch interface in the iPad rendering the page basically broken

    Really my provider tells me that the the only place with tests for Honor Health is in Anthem - an hour away.

  14. Medical Marijuana Card renewal during the Coronavirus Pandemic

    Renewing a medical marijuana card is difficult during this time of CoronaVirus pandemic. For example, my doctor has canceled all non-essential appointments at this time and so getting updated medical papers from her office is not possible at this time. Would it be possible to get an extended renewal date and pay any additional fees online?

  16. Include Additional COVID-19 data now!

    AZ STATE GOVERNMENT: I hope the state of AZ provides more explicit and detailed covid-19 data on Arizona'z covid-19 website. Though you allowed additional data starting 3.29.2020, the website is among the least informative to state residents. As you know full voluntary participation by Arizona's residents is the goal aimed at shortening the rise to the spread of COVID-19. We need to see at least demographics; of active cases, town/cities considered hotspots, hospitalizations and discharges, along with fatalities.     Show a timeline for cases within the State like the one we can see for the entire Country similar to New York State with a linear/logarithmic projection.

  19. The zip code map is very good, however you need more gradations.

    More than 10 doesn't tell much. Maybe have 10 to 50, 50 to 100, 100 to 200 and over 200.
