MyIR Mobile QR Codes
MyIR Mobile QR Codes. I am reaching out directly to our Arizona state health department to request this feature permanently and for more information. Otherwise, the QR code feature will only be available through February 16th. Please make the MyIR Mobile Codes permanent.

Abuse of teachers perpetrated by students/administration
It is hard to find resources if you are a teacher who is abused or who has been abused by students and/or administrators.

"Express Courier" options
Additional shipping options, shown on your website, are available ONLY w/ UPS. I did not know that, checked USPS and could go no farther.
It'd be helpful if "UPS" were stated somewhere near that info. VitalChek was unable to help; she said the "only" options were USPS or UPS.
Once I'd gone back and checked "UPS" instead, I found a small drop down box on a later page that gave me the $17.50 and 19.50 options.

Search by County
The ability to search records by county was previously very helpful when searching for family records. It would be nice if that option was available again.

COVID Dashboard Reporting Frequency
As AzDHS concludes daily COVID reporting, I wish to express my opinion on a job well done. I have used the AzDHS dashboard to track COVID daily since March 8, 2020, when Arizona reported (5) cases.
Through this long and difficult period, AzDHS has evolved the dashboard to meet changing conditions and needs. I also tracked data for several other states, and can proudly say that Arizona’s presentation seemed the most timely, thorough, and coherent of all.

Climate Change CHANGE
I was thinking that someone of like mind may find this message. My ask is to revisit what health encompasses. Climate change is a health crisis, so I figured out how to grow roads. Do you think there might be a way to speak to someone willing to listen to something that is so inexpensive with materials readily available in every community? If we stop spilling a swimming pool of tar at every STOP sign or intersection, and grow roads, the result is being able to run inductive charging throughout the new roads and we would get rid of a…

Login Issue: Repeated attempts to login over last several days gets error "Something went wrong, please try again. If the problem persists, contact support for assistance". I also cannot find the MobileIR app on Google play. What's up with these?

Vital Check is a fraud.
This disgrace of a company has lied to me at least twice regarding my Wife's death certificate.
They have sent back canned and fraudulent reasons as to what "Action Required" was.In each instance they didn't receive the file and straight up LIED as to why....
Why the **** was I tricked into giving these scam artists money and WHY THE **** are you telling people they are legitimate??
Clearly from a simple search they don't do the job!!
Get off your ***** and get a different vendor!! (I doubt you even bother to read these do you?)

trouble finding specific information
Hi there. I previously had a EMT-Basic certification in Arizona and am now a physician applying for state licensure in Pennsylvania. The application requires a "letter of good standing" for any previously held certificates or licenses in the health care field. Would you be able to tell me how to obtain such a document and have it sent to the Pennsylvania State Board of Osteopathic Medicine? Thank you for your time and consideration.

Search function does not work
When I try to search for birth and death records on https://www.azdhs.gov/licensing/vital-records/genealogy/index.php,
a list of results comes up, but clicking on any pdf in the results just returns me to the search screen. This site used to work flawlessly, but now it's pretty much useless. Has something changed?1 vote -
Free ASU Covid Testing be continued
Please consider continuing the free ASU covid testing. It is a resource that is easy to use and the testing allows people at risk to gather with family and friends and be sure that everyone is as safe as possible. It is especially important for people who are at greater risk. The program that has been invaluable is due to expire 6/30

Immunocompromised having trouble getting COVID boosters
I'm immunocompromised and got the 3 primary Pfizer vaccines. When the 3rd primary was approved, I had to go to four locations before I found a pharmacist that was willing to do the 3rd. When it came time for my first booster, I had trouble with Pharmacists thinking my 3rd primary was actually a 1st booster, I had to go to 4 locations. I just got my 2nd booster after a week of trying and being forced to get a letter from my neurologist. Stop outsourcing simple but essential government functions to private corporations that do a terrible job and increase costs.
My son was recently born. Like all parents, we needed a birth certificate to show proof of life so that he could be added to our health insurance. The process to obtain a birth certificate through Vital Check was infuriating.
Navigating their website was simple enough, even though they would have links that just brought you in circles. I uploaded my documents and they came back to say my signatures didn't match on my driver's license.
