    death certificate apllication instuction sheet. on the top of this page it says that the certificates are $20. BUT,on the bottom of the page it states that there is an $8 for processing and a $16 for shipping in addition to the $10 for each death certificate.. so which is it??? $20 total? or, $34 total? this is very very confusing. thank you

    This is a confusing situation to us. The only vendor allowed to sell certificates online for ADHS is www.vitalchek.com. When we went to that website, it does not appear to have the same pricing structure you outlined. Please use only www.vitalchek.com for Arizona records.

  5. i have recently been cut of of my pain medications ( oxycodone) due to my docter refering me to get a card

    how am i to get my pain medications along with the fact i have a medical marajuna card??

    There are no restrictions from ADHS about what doctors prescribe or recommend. You may want to speak to your doctor about your health condition.

  6. I am my brother guardian, he went int Phx to stay with friends son while that person went into hospital for cancer screening.

    How does a person who is a family members guardian even find out if they have been placed in the hospital ? 43 days now, no cell phone, no medicine.Not in jail, not dead, but his is bi polar.

  7. It should not take 2 months to receive a death certificate. It is more grief and hardship for the family.

    The amount of time it takes depends on many factors. We understand that it is a difficult time and try to make the process as simple as possible.

  8. Question about S-LP Licensing Requirements

    Hello,
    I am a Speech-Language Pathologist interested in working in Arizona in the future.
    I hold dual citizenship with Canada and the USA. I have two years of experience, am currently certified by CASLPA, and am in the process of becoming certified by ASHA.
    My question is regarding the Praxis exam - under the "Mutual Recognition Agreement", I am able to gain ASHA certification without writing the Praxis exam, as I have already passed the very similar CASLPA exam. However, I note that under your State licensing requirements, the Praxis exam is listed.
    I would appreciate your advice about this…

  9. Get my medical marijuana card to me, applied May 24, 2016, paid for, approved and today (July 26, 2016) I still do not have it.

    If mdAnderson is right, I may have up to 5-6 months to live. I have 9 deadly cancer and applied for a marijiuana card May 24th. Never smoked a cig. in my 72 years, never drank or did drugs and am scared to death of what I have coming. Shouldn't have to take from May 24th, 2016 to and BEYOND July 26th, 2016 to get a card.
