you morons denied my request in 2010 for death certificate of my grandfather, i was searching for his place of death.
in 2012, i located his monument in the glendale cementary,, no fee/ no

Legal dispensaries need to be listed. The card and doctor costs money, but no where to get the product.
Dispensaries need to be listed listed clearly on websites, including state health departments.
1 voteUnder the voter-passed initiative, the Department is not allowed to share any information about dispensaries except to registered cardholders. You can email m2programsupport@azdhs.gov with your registration number and staff will help you.

Bring back your old site with information on the address & licensing for assisted living facilities!
Bring back the searchable website. How do I find licensed assisted living facilities within a zip code?

Thanks for writing. This is a good point and something that we are linvestigating. We are evaluating all of the possibilities with our IT department at this time and may be changing it in the future.

Go back to a searchable database rather than reports I have to scan through
Return to the website that can search for licensed facilities by name, address, zip code etc. This new list retrieval is difficult at best to work with. It takes me almost double the time to find a facility and that's if it is in a small zip code area!
1 voteWithout a better explanation, we can’t help you much. You can search facilites by name, address or zip code through azcarecheck.com or you can download the full listing of each type of facility from the facility and provider listings. We haven’t made any changes to our listings recently. Feel free to call 602-542-1094 if we can help you though an issue.

Website for information use is difficult to use.
It was much easier to use before you changed it to do with licensing for Assisted Living and Long Term Care. Sometimes web sites are just fine and don't need the extra hoopla. Thank you.

DNR bracelet
Your requirements or SO specific, perhaps a link to where a person could acquire a bracelet is in order.

An option to receive e-mail or text alerts on RFP awards would be helpful.
A place to enter my email or mobile # so RFP award notifications that are announced are sent immediately would be a nice feature.
1 voteMaybe something like, “That’s an interesting idea, but we do not have that type of service available at the state right now. The state’s procurement system is set up through https://Procureaz.gov. You can always check there for new information.”

Smell of Feces In Carl's Jr on SW Corner & Thunderbird & 51st Avenue's
The Carls Jr on 51st Avenue has a super strong smell of Feces in the Mens Public Bathroom. It is so strong you don't even want to be in there even the few seconds I was in there yesterday at 5 PM.
Today is Monday, 6:40 PM
When I told the Manager he noncholantly said "Its been there a while". He didn't say he would clean the bathroom up or scrub everything in there like I suggested. He just took it in stride as if it was an everyday occurrence. Similar to the Southwest Stores' Rotten Meat Smell,

PM Attachment 7.5.4 appears to be an error
What’s linked for 7.5.4 (Table) is not what’s described in 7.5.4 (Preamble). It’s just an ICD-9 description.
1 voteThe Provider Manual Attachment 7.5.4 was replaced a few years ago from the preamble to the Substance Abuse Disorders Qualifying Diagnoses Table. The Qualifying Diagnoses Table is utilized to identify the Behavioral Health Category Assignment (see the Demographic and Outcomes Data Set User Guide) of individuals who are being served in the public behavioral health system. The Behavioral Health Category Assignment is part of the Demographic data that the Division of Behavioral Health Services uses to create numerous reports, which may include descriptions of the populations served within the public behavioral health system and the services being utilized by these populations.

That’s an interesting suggestion. At this time we do not have any plans to provide that service. Depending on when you ordered the copy of your birth certificate, it may be on its way. In Washington, Colorado and California the costs are much less for the intake and use of medical marijuana compared to Arizona.
It is a proven fact that the use of marijuana has less side effects than some of the medication that are prescribed for pain, nausea, nerve problems and even some of the phych. medications.
Medicalll marijuana is not for everyone, though those that it does work for to ease the medical issues should not have such a higher price to pay.1 voteWe appreciate your feedback. When the voter initiative was passed to begin the Medical Marijuana program in Arizona, there was no money provided. Prices were set to prevent all taxpayers from paying the cost for those who use the program. The department does not have any jurisdiction concerning the cost at dispensaries or whether insurance pays for marijuana.

STATE MONEY
IN THE BEGINNING OF THE MMJ PROGRAM THERE WAS A BLURB ABOUT A STATE ALLOTMENT FOR PATIENTS WHO CANT PAY THE FEES, WHAT HAPPENED TO THAT? IS IT STILL THERE AND HOW DO THE PATIENTS APPLY?
1 voteThere was never any state money tied to the medical marijuana program. All the money has been generated from card holders and dispensary applications. The provision you may have been remembering was the discounted card fee for people who received SNAP benefits.

LOST CARDS
WHY IS IT WHEN PATIENTS LOOSE CARDS THEY NEED THAT LONG # TO REPLACE IT???? IF ITS A LOST CARD ITS NOT AROUND, SO MANY OF THE PATIENTS CANT REMEMBER ANYTHING AT ALL ON A DAILY BASIS, HOW ARE THEY SUPPOSED TO REMEMBER THAT LOOOONG # !?

There is no official list of officially licensed AZDHS Dispensaries.
Please provide a list of officially licensed AZDHS Dispensaries that is easily accessible on the website. Thank you.

Where are the dispensaries? Is there only one?
I am a stage four cancer patient. The medical marijuana law passed three years ago. I may be dead before Arizona has a program in place. If you tried, could you make implementation of a program any slower?

Perhaps you could devote a page on your website that shows the name and location of licensed and approved dispensaries
Could you post a list of licensed dispensaries, with addresses, for us cardholders, so we can find a source for our medicine?
1 voteUnder teh voter approved initiative, we are not allowed to disclose information about the dispensaries to anyone except cardholders. You can write to m2programsupport and we will email a list to you if you are a cardholder.
