To obtain the birth certificate of my grand mother, I need the date of her birth, but I only know the year, what can I do?
Hello! I only know the year of my grandma´s birth, what can I do to obtain her birth certificate?
1 votePlease complete the application with as much information as possible so that a search for the record can be conducted. We will make every effort possible to locate the record based on what you provide in the application.
Include the names of the parents and the county where the registrant was born if this information is available.If a record is located and you meet the eligibility requirements, a certified copy will be issued to you. If we are unable to locate the record, a certificate of no record will be issued. Please see our website for the eligibility requirements for requesting a birth certificate.

LGBTQ Series Recordings
At one time through the Office of Prevention website, there was a list of recordings on various topic pertaining to gay, lesbians, *****, and the transgender community. None of the links work. When looking for state statistics on health disparities for this population I could find nothing under all the categories offered under A-Z. I hope that was an error on my part and the information is there. With the emphasis on integrated care in both medical and behavioral health settings, addressing explicit and implicit bias in healthcare settings is vital, as is data collection for this subgroup minority…
1 voteThe Arizona Department of Health Services (ADHS) understands there are unique health challenges that the Lesbian, Gay, Bisexual and Transgender (LGBT) community faces; such as reduced access to health care and insurance, and lack of cultural competence by providers. While there is limited health-related data collection on sexual orientation and gender identification, the ADHS is committed to creating health equity and ensuring optimal health for the LGBT community in Arizona. Current efforts include designating the LGBT population in Arizona as a priority in efforts aimed at preventing and reducing the prevalence of tobacco use and HIV/AIDS.

AZ state med marj program rep to provide program and discounts for Combat Vets renewals w/docs being able to file w/state rep saving time locating MDs in rural areas ie Casa Grande. How about this suggestion for AZ State County in disp Reps and discounts for reapps Combat Vets. Thanks for your Support

No one seems to know just what "incidental marijuana" is. What is it?

not easy to find
I need my middle name put back on my Birth Certificate, to do this a need a certified copy of court order with raised seal. I was born here in Kingman [at the old Kingman Hospital] Mohave General Hospital

On the first page to describe the CEU, create text box which directs user to scroll to the right to complete the submission of a CEU.
Allow access to the CEU records DHS has already recieved and does not need to be resubmitted on this form....eg. ASHA CEUs, ArSHA CEUs, ADE Directors Institute CEUs.

I have my amend papers birth certificate add middle name, ready to mail. Where do I mail it to?
Can I also amend here in Mohave County Department of Public Health Vital Records?1 votePlease visit azdhs.gov/licensing/vital-records and click on the tab that says “corrections and amendments.” There will be an option to correct a birth certificate.

vital records question - proof of applying for a birth certificate
Hello,
I would like to obtain a statement showing when an application for an infant's birth certificate was submitted to Pima County in 1985. I wish to show the passport office that the birth certificate was applied for in in a timely manner after the birth. How would I get a written statement from your office?

Complaint
The dispensary agent in Holbrook, AZ Swell Farms has hired an individual that has close ties to an illegal methamphetamine operation in town. License plate # BCE-3333. She a daughter of Gina Gabaldon who lives at 853 N. 3rd Ave, Holbrook, AZ. Her mother operates on the largest Meth. house in Holbrook, and has for years. Check with MCAT, with the Sheriff's dept. She visits that house often. Swell Farms also engages in unfair and unethical business operations; I was discharged after 3 hours of work because of false accusations made known to management by this employee. Please contact me…

I need a certification of my medical report by the AZ Depertment of Health for my visa application for Saudi Arabia. Unfortunately there is no email contact option for such a relevant question. Instead I find a lot about Marijuana or email spam subscription as from a commercial company. There needs to be a general enquiry option for such serious medical/administrative questions.
