Vital chek is not worth it
This is not the fastests option. They will say that your ID wasnt acceptable even when its a clear copy uploaded not faxed. Waste of time and the extra money that you pay thinking its going to speed up the process

When applying for a birth or death certificate through our expedited vendor it’s important to remember that if the application is incomplete or unacceptable, your order will be delayed and you will not receive your certificate in an expedited manner. Several items may cause a delay in the expedited process including unacceptable identification. Uploading you identification versus faxing your identification is preferable because a faxed copy of identification may not always come through clear on the other end. If you upload your identification, there may be other problems that could delay your order. Some of these problems are, an expired identification, the identification appears to be damaged, faded, pealing, or cracked, the signature on the identification does not match the signature on the application, or the signature on the…

MEASLES VACCINATION CAN AVOID DEAFNESS IN CHILDREN
YOUR PULIC SHOULD BE APPRISED THAT IT IS POTENTIALLY POSSIBLE THAT THE MEASLES VIRUS COULD ATTACK VARIOUS GERMINAL TISSUES IN THE BODY OF YOUNG CHILDREN. MY BABY SISTER, AT AGE 18 MONTHS IN THE 1940'S, HAD MEASLES. MY MOTHER TREATED THE HIGH TEMPERATURE. SHE BECAME TOTALLY DEAF. THE PUBLIC DOES NOT KNOW THE POSSIBLE RISKS LIKE THIS. AS AN EMERITUS PROFESSOR THAT TAUGHT DEVELOPMENT FOR 37 YEARS, I HAVE EXPERIENCE IN SUCH DEVELOPMENT-RELATED PROBLEMS. YOUNG PARENTS SHOULD BE EDUCATED ABOUT SUCH POSSIBILITIES. PLEASE CONSIDER BROADCASTING SUCH POTENTIAL EFFECTS!!!!

If I can grow my own and want a care giver,do I have to change my license.
My license say that I can cultivate, but I would like to have a caregiver right now, do I have to change my license.

Misspelling of Arizona Children's (spelled Chidlren)--for Apache Junction, Coolidge, Flagstaff etc.
Correct spelling in all areas where the word appears.

Site is confusing. Navigation doesn't match newsletter.
Please put more clear instructions on the change of address form. Am I supposed to enter my current address (for verification) or my new address? And why do I need to upload a scan of my ID? My ID isn't updated yet, so what good is that? Also, the MMJ newsletter I just received in the mail says to go to azhealth.gov/medicalmarijuana and click on "card change application". Well there is no such link. One must first click on "Applications", then click on "Card Change Application", which of course is very confusing.

HELP - phone number 888-816-5907 options are in a loop
I am trying to contact a customer support agent and when i click the option number(5) it returns me to the main menu. Nothing I can do will get me to a customer support agent.

my brother died in az 1-16-2015 how do i get his death record
I live in Michigan and am broke and I need the certificate for his insurance threw general motors I did't know he was that ill he never told any family, his insurance sent me a letter. and they said he died their should be a better way than a letter.

How about giving Ajo back its grow rights seeings how they are not trying to open the dispensary doors ! How come they can grow and we can't. we can't even keep our care giver what about the poor people or is this only for the rich ?

Require Vitalchek to follow published instructions
My son was not issued a birth cert after following instructions provided by Vitalchek. I was charged for a birth cert that they will not process without a state issued Id. The notarized form was received by them. I must now file as a parent because he needs the bc for a state issued id.

Seeings how the dispensary in Ajo hasn't opened or hung out a sign or sold a single gram in 20 days how about giving us back our grow rights !

Look into the populus who were raised in Morenci AZ in the 60's, 70's and 80's .
There have been a multitude of my former classmates who either died young and still lived in Morenci or who have moved away and have passed from some form of cancer or are currently suffering, or are survivors of this disease. There has to be a correlation to this. There are too many occurences. Either caused by the copper mines air quality at that time or the occurrence of radiation fall out in that area.

Information on site is unhelpful
why not provide an easy way to access records (in particular marriage and divorce) - looking for two divorces, one in about 1919 and another in 1946 or 47. Have no idea how to start, other than paying a service to do this. Your on-line records for older birth and death certificates works well. Why not do the same with other records?
1 voteMarriage and divorce records are not available from the Arizona Department of Health Services, Office of Vital Records. Marriage and Divorce records in Arizona are maintained by the Clerk of the Superior Court in the county where the event occurred.

close the dispensary in Ajo
Day 24 and still not open ! Can we mail in a petition? A lot of card holders can't afford a computer . It cost me $300 the day it said it was opened I only make $550 a month on ssi . I thought this program is for the poor

open the vital records office to the public!!!!!!
This office is essential! There are instances when time is an issue and without being able to get immediate assistance things take an immediate turn for the worse and live are greatly impacted for the worse. This comes from personal experience. Yet again our government failing us and our taxes are for nought.
