Where do I let you know a store is keeping out of date items
Safeway on University and Lindsay has items out of date on shelves constantly, may yesterday had January 2017 date on it is now May2017 took it back all on shelf out of date, customer service not worried said thanks, have found many items out of,date, do,we have to,get,sick before they do,their job, we should not be doing it for them. Also constantly milk is bad even with good sell by dates, what
is wrong with this store, you need to check this store out to make us all safe.1 voteFood is not required to have an expiration date. The date on the packaging has more to do with product quality than food safety.
Food products (regardless of whether they are expired or not) must be wholesome and unadulterated when offered for sale.
Any concerns regarding products can be directed to the agency that permits and inspects the store, which in this case is Maricopa County Environmental Services.
Complaints can be filed by visiting esd.maricopa.gov or calling 602-506-6616.

Contact information requested
I am looking for an answer to a general question and can't find the appropriate department. The question is: Is it a state law that a Human Hospital Laboratory cannot process a lab test on Canine blood (such as a progesterone test). If anyone can answer my question, or direct me to the appropriate department/statute, I would appreciate it! Thank you in advance.

cover up on valley fever
I have a sister who has battled 9 years of valley fever-she has had a stroke and heart attack-she is in a study with NIH in Bethesda as a case they have never seen-why does Arizona cover up this deadly dust disease with no cure? I am disgusted that today my sister is back in the hospital because she is so sick-9 years she is fighting a fungus that Arizona covers up ! I am disgusted! we have deadly air that no one ever wants to address because Arizona has so many tourists ! I am ready to report this…

Pima County has 17 Drop boxes
You have the Tucson Police Dept Drug drop boxes but not the Pima County Sheriff - Tucson and Green Valley.
UMC - Banner Main also has a box for a total of 17 boxes in Pima County.

I am attempting to order a copy of my sons birth certificate. The only option for shipping is 18.00 airmail. Like you I am not home during the day, I am working. I have a locked mailbox and I do not understand why I am paying 18.00 to ship a piece of paper from Phoenix to Scottsdale?
1 voteADHS, at this time, does not have web-ordering available for general issuance. We offer web-ordering for expedited service thru our vendor Vital Check that does charge a shipping fee. You may apply in person at the Maricopa County Office of Vital Registration, they have multiple valley locations.

Hello, I wanted to find out if tattooing was regulated. What are the requirements?

birth certificates out of state
why is it i can order from a third party and get certified copies overnight but can't get one from you?

can you connect with other states vital records to get a certified copy. records office to records office if picked up in person by authorized agent.

The web page http://azdhs.gov/documents/preparedness/epidemiology-disease-control/disease-integrated-services/adap/adap-application-english.pdf currently loads an OUTDATED version of the Ryan White/ADAP application. 1 voteThe application on the website is in effect until December 31, 2016. The ADHS HIV Care and Services Program has been piloting a new application. During the pilot period, clients and service providers have given feedback on the ease, structure and requirements of the application. Their feedback has been reviewed and incorporated into a final version of the application. A Spanish translation was created based on the final application. The new application will be effective on January 1st and will be posted to the website on December 30th.

Please put details regarding proper disposal of cigarette butts in your Tobacco Free AZ brochures
It would be helpful and terrific if you could put a couple lines in these nice brochures you offer about how to properly and safely dispose of cigarette butts, and that it is considered littering to leave butts on the ground.
