    We cannot post one fee schedule on the front page because fees may be different depending on where you apply for the certificate. There is a page that lists the fees for birth certificates linked off the same page as the application.

  4. What the Heck????

    My Mom passed in 1988. What a mess to get a death certificate. I do not know the date and month, that was 25 years ago. I can provide all of the information if anyone would pick up a phone and listen. And do not tell me that you get thousands of requests every day. This is a personal matter, and being treated like some terrorist appals me. It is about my MOTHER. Thank You for your lack of concern, and lack of service to your public.   6. Genealogy Death Records

    I've attempted several times to search for my relatives death record. Carl G Holz died 19 May 1908 in Maricopa County Arizona. Each time I begin the search, the website closes my web browser.
    Can you assist me, please? I appreciate your assistance.

  7. index of marriage records by name

    I am looking for my grandparents marriage license. They were possibly married in Phoenix in 1929. Isn't there a data base somewhere of names and marriage info. to at least learn if they were married here?

    The State Department of Health Services is not the keeper of the marriage license records. We do keep a total number, but you would need to check with the County in which they were married for more information.

  8. open the vital records office to the public!!!!!!

    This office is essential! There are instances when time is an issue and without being able to get immediate assistance things take an immediate turn for the worse and live are greatly impacted for the worse. This comes from personal experience. Yet again our government failing us and our taxes are for nought.

  9. Information on site is unhelpful

    why not provide an easy way to access records (in particular marriage and divorce) - looking for two divorces, one in about 1919 and another in 1946 or 47. Have no idea how to start, other than paying a service to do this. Your on-line records for older birth and death certificates works well. Why not do the same with other records?

    Marriage and divorce records are not available from the Arizona Department of Health Services, Office of Vital Records. Marriage and Divorce records in Arizona are maintained by the Clerk of the Superior Court in the county where the event occurred.

  10. Look into the populus who were raised in Morenci AZ in the 60's, 70's and 80's .

    There have been a multitude of my former classmates who either died young and still lived in Morenci or who have moved away and have passed from some form of cancer or are currently suffering, or are survivors of this disease. There has to be a correlation to this. There are too many occurences. Either caused by the copper mines air quality at that time or the occurrence of radiation fall out in that area.

  11. Require Vitalchek to follow published instructions

    My son was not issued a birth cert after following instructions provided by Vitalchek. I was charged for a birth cert that they will not process without a state issued Id. The notarized form was received by them. I must now file as a parent because he needs the bc for a state issued id.

  13. my brother died in az 1-16-2015 how do i get his death record

    I live in Michigan and am broke and I need the certificate for his insurance threw general motors I did't know he was that ill he never told any family, his insurance sent me a letter. and they said he died their should be a better way than a letter.

  14. HELP - phone number 888-816-5907 options are in a loop

    I am trying to contact a customer support agent and when i click the option number(5) it returns me to the main menu. Nothing I can do will get me to a customer support agent.

  15. Vital chek is not worth it

    This is not the fastests option. They will say that your ID wasnt acceptable even when its a clear copy uploaded not faxed. Waste of time and the extra money that you pay thinking its going to speed up the process

    We apologize for your frustrating experience and would like to offer some explanation as to why you may not be satisfied. When applying for a birth or death certificate through our expedited vendor it’s important to remember that if the application is incomplete or unacceptable, your order will be delayed and you will not receive your certificate in an expedited manner. Several items may cause a delay in the expedited process including unacceptable identification. Uploading you identification versus faxing your identification is preferable because a faxed copy of identification may not always come through clear on the other end. If you upload your identification, there may be other problems that could delay your order. Some of these problems are, an expired identification, the identification appears to be damaged, faded, pealing, or cracked, the signature on the identification does not match the signature on the application, or the signature on the…

    Arizona is a closed record state and only certain people can have copies of birth certificates. Because of that there are certain steps to go through to have a copy. Please visit the website for more information: http://www.azdhs.gov/vital-records/birth-certificate/obtain.htm
    If you qualify, you can reach out by mail order or go to a county vital records site to have one made.

  17. Please provide a listing of ALL deaths (obituaries) by date.

    I can't find a listing of all obituaries or notices of death, by individual. The Republic provides PAID obituaries but there are a many other decedents that should have notice of passing as part of the public record. Having a search function, by name, presumes one knows who has passed on. A complete listing of deaths, by date and name, is sorely needed.

    We cannot help you with this request. Arizona is a closed record state. We are not allowed to release information about deaths except to specific individuals.

    Arizona is a closed record state. Only certain people are able to obtain copies of birth and death certificates. We do have an excellent website for death records that are old enough to be released: http://genealogy.az.gov/

  20. Have received UNCERTIFIED copies of Birth Certificate/NEED CERTIFIED COPY

    I am writing on behalf of an 18 year old student who has been trying to obtain his certified birth certificate. He has sent in requested document/money and receives uncertified copies. As a result he cannot obtain his driver's license in Tx. Could someone please clarify this?

The State of Arizona is considered a closed record state so we are unable to release specific information to you unless you meet the eligibility requirements.
